What you will do

• Carry out the local authority statutory requirements under The Care Act 2014, The Mental Health Act 1983/2007, and The Mental Capacity Act 2005.
• Manage a caseload of individuals with complex mental health needs.
• Support timely hospital discharges, addressing and overcoming barriers.
• Conduct Section 117 aftercare planning and reviews, ensuring appropriate services are arranged.
• Arrange and coordinate care packages, placements, supported accommodation, and community support services to facilitate safe hospital discharges.
• Engage with staff across the Council, customers, carers, and various agencies, including statutory and voluntary services.
• Maintain high professional standards in line with national occupational standards.
