Senior Staff Nurse

Are you an experienced Senior Nurse within an acute care setting looking for an opportunity to utilise your proven leadership skills alongside patient clinical assessment and acute care delivery? If so, this unique role is for you.

The Acute Care at Home service is a partnership between the University Hospital of North Midlands (UHNM), University Hospital of Derby and Burton (UHDB), Royal Wolverhampton Trust (RWT) and Midlands Partnership Foundation Trust (MPFT). The successful applicants will join an integrated, collaboratively focused team, employing proactive clinical practice and knowledge, to provide clinical assessment, monitoring and support, as part of our urgent care pathways.

Due to an appointment within the service, we have an exciting opportunity for an experienced nurse to join our Central Virtual Ward team.

Main duties of the job

You will be based at County Hospital and will play a key role in supporting the Central Virtual Ward team in hospital admission avoidance and supporting patients to leave hospital sooner than would otherwise be possible.

You will provide senior leadership working closely with the Clinical Nurse manager, visiting patients in their own home to carry out clinical assessments, formulate treatment plans and provide evidence-based care. You will also spend time as shift coordinator; facilitating staff support, triaging new referrals and liaising with members of the MDT to ensure patient safety and service continuity to the standards required.

Job responsibilities

Utilising Physical Health assessment skills to deliver direct clinical assessment and care provision of people in their home environment.

Co-ordination of daily Multi-Disciplinary Team meetings and Consultant ward rounds, ensuring that patient related information is up to date and relevant clinical information is disseminated clearly and concisely.

Act as a point of contact providing advice, support and guidance to patients, colleagues and other healthcare professionals on issues relating to patient care.

Communicating sensitive information concerning patients medical condition(s) and status which involves relationship building; displaying persuasive and reassurance skills acting in the best interests of patients at all times.

Qualifications
  • NMC registered
  • Physical Health assessment
  • Full Driving Licence
Experience
  • Evidence of leadership skills
  • Evidence of utilisation of Health assessment skills
  • Evidence of mentorship/teaching in a clinical setting
  • Experience working at band 6
  • background in acute medical care setting in NHS
