Job Description

We are hiring a Senior Clinical Services Pharmacist to our Hospital in Dublin, Ireland.
Job Type: Permanent
Working Hours: 35 hours over 5 days or 32 hours over 4 days
Flexible Contract Options: Candidates can choose between a 5-day (35-hour) or 4-day (32-hour) working week.
Key Responsibilities
- Deliver comprehensive clinical pharmacy services to ensure the safe, effective, and evidence-based use of medicines.
- Review and optimise medication therapy to improve patient outcomes and minimise medication-related risks.
- Participate in multidisciplinary team (MDT) meetings and provide expert pharmaceutical advice to healthcare professionals.
- Conduct medication reconciliation, clinical screening, and pharmaceutical reviews for patients across designated clinical areas.
- Monitor patients for adverse drug reactions, medication interactions, and therapeutic effectiveness, recommending appropriate interventions.
- Support antimicrobial stewardship, medication safety, and quality improvement initiatives throughout the hospital.
- Develop, implement, and review clinical pharmacy policies, protocols, and standard operating procedures.
- Supervise, mentor, and provide clinical guidance to pharmacists, pharmacy technicians, interns, and students.
- Participate in the education and training of pharmacy staff and other healthcare professionals.
- Promote evidence-based practice by contributing to clinical audits, research, and service development projects.
- Be registered with the Pharmaceutical Society of Ireland.
- Hold an MSc or Diploma in Clinical Pharmacy, Hospital Pharmacy, or an equivalent postgraduate qualification.
- Have a minimum of 3 years' post-registration clinical pharmacy experience in a hospital setting.
- Demonstrate strong clinical knowledge and expertise in the safe and effective use of medicines.
- Possess excellent communication, organisational, and interpersonal skills with the ability to work collaboratively within a multidisciplinary team.
