Consultant Cardiologist Sligo University Hospital for 18.5 hours per week and HSE West & North West region for 18.5 hours
This is an appointment to the HSE West and North West on a Public Only Consultants' Contract 2023 by the Health Service Executive. The initial commitment for this post will be to Sligo University Hospital for 18.5 hours per week and HSE West & North West region for 18.5 hours. This post may be subject to restructuring in the future to facilitate the reorganisation of acute services in line with new clinical models of acute and community services. It is noted that the post must be congruent with the requirements of, and facilitate implementation of national health policy, the HSE's National Clinical Programmes including commitment to deliver the relevant performance outcomes.
Due to the establishment of the HSE Health regions and the geographic realignments of the CHO Areas/Hospital Groups within these regions, the appointing service may differ from that outlined in the Letter of Approval for this post
Main Duties of the JobThis new post will drive integration in ambulatory care between the hospital and community services. The post holder will work with colleagues across these services to develop and implement ambulatory care pathways to manage cardiovascular diseases within the community setting. This post will involve ongoing engagement with the Heart Failure CNS and CANP already in place, and engagement with the multidisciplinary team in the Community Hub to provide access to community diagnostics, care and timely specialist opinion.
The Heart Failure Virtual Clinics commenced in March 2022, and continued development of this service is anticipated.
The Integrated Care Consultant will work 50:50 between the Ambulatory Care Hub for Chronic Disease Management Sligo and Sligo University Hospital and will work to develop the alternative modernized outpatient pathways in their specialty in their designated hub. This model of care supports the provision of health care as close to home as possible. It provides access to community diagnostics, community specialist multidisciplinary team services and timely specialist opinion within the specialist ambulatory care hub in order to support the provision of care at the lowest level of complexity, as appropriate.
As outlined above, the need to reform the healthcare services in Ireland in order to provide a more sustainable, integrated and patient-centred approach has come to the fore in recent health policies and strategies.
Integrated care requires health and social care services to work together across different levels and sites in order to provide end to end care that meets patient need. As described in the Sláintecare report (2017), integrated care involves:
- Ensuring appropriate care pathways are developed with a focus on person centred service planning to ensure services are built around patients;
- Supporting timely access to all health and social care services according to medical need Patients accessing care at the most appropriate, cost effective service level with a strong emphasis on prevention and public health.
- The Cardiology Consultant, Lead in Integrated Cardiology Care will lead such a service for those with chronic cardiovascular conditions, with an emphasis on providing care within the ambulatory care hubs. This is a new full time post, designed to support the development of cardiology medicine as part of integrated care.
The Consultant will provide leadership in the provision of chronic disease ambulatory care within the ambulatory care hub and support the development of integrated services across the wider region. This post will support the implementation of Sláintecare in Ireland and represents a unique opportunity for the successful candidate to be at the forefront of this new way of working, influencing, shaping and leading the roll out of integrated cardiovascular care in Ireland.

Information about the organisationThe Enhanced Community Care Plan was developed by HSE Community Strategy and Planning together with the Integrated Care Programme for the Prevention and Management of Chronic Disease (ICPCD).
Funding has been allocated to roll out and scale up community healthcare services to support the provision of integrated end to end care. The ICPCD is supporting a model of integrated care that has a particular focus on preventive healthcare, early intervention and supports to live well with chronic disease.
The implementation of integrated care is dependent upon the development of community based, multidisciplinary roles. The Integrated Care Consultant will lead the Chronic Disease Community and Hospital Specialist Multidisciplinary Teams supporting the clinical service design, implementation and clinical governance across both the hospital and community sectors in their designated region.

The annual salary will be as set out in the Public Only Consultants' Contract 2023. Medical Consultants Salary Scales from 1st February 2026:
Incremental credit is awarded in respect of previous experience at Consultant level.
Pro Rata Sligo University Hospital for 18.5 hours per week and HSE West & North West for 18.5 hours.
