Job responsibilitiesAbout the role
Working within the Specialist Palliative Care Home Team, you will provide senior medical leadership and expert clinical care to patients in the community. You will advise on and lead complex symptom management, advance care planning, treatment escalation planning and support for patients and families. You will work collaboratively with multidisciplinary teams across primary care, secondary care, social care and the voluntary sector.
You will also support the twice-monthly outpatient Pain Management Group alongside the Consultant Anaesthetist and Psychological Therapist, and take a lead role in developing the hospices research, evaluation and quality improvement agenda.
Key responsibilities
Assess and manage complex physical, psychological, social and spiritual symptoms in community settings.
Provide clinical support and advice to GPs, district nurses, care home staff and community teams.
Lead and support advance care planning, DNACPR discussions and treatment escalation planning.
Work closely with the Specialist Palliative Care Home Team Lead to develop and strengthen the service.
Participate in multidisciplinary meetings, case conferences, governance activity and clinical education.
Lead and promote research, audit, evaluation and quality improvement across the hospice.
Support resident doctors, hospice staff and external healthcare professionals through teaching and clinical supervision.
Contribute to the second on-call rota shared across the hospice, CUH and RPH.
About you
We are looking for a consultant who brings excellent clinical skills, compassionate leadership and a strong commitment to person-centred palliative care. You will be confident working with patients and families with complex needs and able to build positive relationships across professional boundaries.
To be successful, you will need full GMC registration and CCT/CESR or Specialist Registration in Palliative Medicine, or be within six months of achieving this at interview. Experience across palliative medicine settings is essential, and experience in a community hospice post would be advantageous.

Starting salary of £113,56, with opportunities to progress to £150,569 (Consultant medical grade with 1% on-callbanding).
Please note that the salary quoted is based on full time hours and will be pro-ratad for part time posts. As a Charity with limited funds, it is usual practice for new starters to commence on the minimum of the salary band for the role. Exceptionally, and only for candidates demonstrating significant strengths relevant to the role, we may be able to negotiate on this.
