Job Description
Responsibilities 
- Act as a patient's key worker and support coordination of patient care along the treatment pathway.
- Ensure patients are offered holistic needs assessments at relevant points throughout the pathway.
- Assess patient information needs and meet those needs as required.
- Clinically review and assess cancer patients in the Outpatients setting.
- Use professional and clinical skills to act as resource and advocate for patients, promoting holistic wellbeing of patients, family and carers.
- Work as a core member of the multidisciplinary team, supporting the cancer pathway to improve patient experience.
- Deliver evidence based individualized patient care according to patients' changing health needs through Holistic Needs Assessment.
- Understanding of the needs of cancer patients.
- Excellent communication skills for advising and supporting patients, relatives and other health professionals.
- Knowledge of managing a range of conditions, including disease progression complications and side effect management of systemic anti cancer treatment and radiotherapy.
- Ability to speak Welsh desirable; Welsh and/or English speakers welcomed.
