Job Description

Responsibilities

• Act as a patient's key worker and support coordination of patient care along the treatment pathway.
• Ensure patients are offered holistic needs assessments at relevant points throughout the pathway.
• Assess patient information needs and meet those needs as required.
• Clinically review and assess cancer patients in the Outpatients setting.
• Use professional and clinical skills to act as resource and advocate for patients, promoting holistic wellbeing of patients, family and carers.
• Work as a core member of the multidisciplinary team, supporting the cancer pathway to improve patient experience.
• Deliver evidence based individualized patient care according to patients' changing health needs through Holistic Needs Assessment.

Qualifications

• Understanding of the needs of cancer patients.
• Excellent communication skills for advising and supporting patients, relatives and other health professionals.
• Knowledge of managing a range of conditions, including disease progression complications and side effect management of systemic anti cancer treatment and radiotherapy.
• Ability to speak Welsh desirable; Welsh and/or English speakers welcomed.
