Job Description
The successful postholder/s will work across our 3 Southend Coastal Site in Shoeburyness, Thorpe Bay and Westcliff.

Rates of pay are negotiable based on previous experience.

Full & Part-Time hours are available.

Advanced clinical practitioner masters is essential.

The post holder willbe practising as an Advanced Clinical Practitioner (ACP) within theirdesignated specialty/directorate. The role will encompass the skills ofassessment, examination, diagnosis, and treatment within an agreed scope ofpractice. The post holder will assist in the safe assessment, treatment,referral, and discharge of patients in their designated directorate.

Main duties of the job The post holder will:

Work towards health promotion and prevention andcomprehensively assess patients for risk factors and early signs of illness.

Draw on adiverse range of knowledge in their decision-making to determine evidence-basedtherapeutic interventions, which will include prescribing medication wherelegally allowed and actively monitoring the effectiveness of therapeuticinterventions.

Plan andmanage complete episodes of care, working in partnership with others,delegating and referring as appropriate to optimise health outcomes andresource use, and provide direct support to patients and clients.

Work inpartnership with patients and support them to manage and live their lives.

In addition, they will help todevelop a programme of practice development for staff Primary Care who willalso be caring for these patients. His or her practice should be developed toan advanced level whereby the Advanced Clinical Practitioner can directly orindirectly influence all aspects of care and management of patients withintheir speciality, and across a population.

About us Southend Coastal Surgeries work across 3 sites in Southend, Shoeburyness, Thorpe Bay and Westcliff. We implemented Anima full digital triage over a year ago at our Shoeburyness and Thorpe Bay sites and have recently implemented Anima at our Westcliff site.

Working together inpartnership to deliver services to their patient population across bothpractice sites, the practice is clinically lead by a GP and an AdvancedClinical Practitioner, the partners are supported by a diverse clinical teammade up of salaried GPs and part time locum GPs, Advanced Nurse Practitioners,Associate Nurse Practitioners, Prescribing Paramedics, Practice nurses, GP NurseAssistant, Health Care Assistants, a Lead Pharmacy Technician, PharmacyTechnicians, and a Prescription Clerks.

Job responsibilities Clinical

Uses specialistknowledge and specific training to act autonomously in assessing, planning, andimplementing a comprehensive management plan within their defined scope ofpractice.

Elicit apatient history appropriate to the clinical situation, which will include,presenting complaint, history of the present illness, past medical history,social history, family history, medications, allergies, review of systems, riskfactors and appropriate targeted history.

Perform aphysical examination tailored to the needs of the patients and the demands ofthe clinical situation.

Assess andprioritise patients undergoing treatment.

Demonstratecontinual evaluation of the patients and use expertise to recommendadjustments/amendments to treatment plans, in consultation with the patient andmembers of the multi-professional team.

Make directreferrals to other members of the health care team.

Requestsbloods and radiological test as appropriate and within agreed scope

Request andperform diagnostic tests as agreed locally and within agreed scope.

Chase, viewand Interpret results and report findings.

Discuss andagree assessment outcomes with patients, carers, and other health careprofessionals to enable patients to make an informed decision regarding theirtreatment.

Communicatehighly complex information to patients, carers and families during the consentprocess prior to commencement of treatment plans and invasive proceduresincluding explaining treatment options.

Authorisethe decision to admit patients and/or proactively initiate discharge.

Activelysupports patients and their families through communicating difficult and highlycomplex sensitive information using a variety of methods to ensure effectiveunderstanding in a timely manner.

Proactivelysupports patients, carers, and families in coming to terms with theirillness/condition.

Providesempathy and reassurance through understanding the quality-of-life issuesassociated with their illness/conditions and treatment related side effects.

Ensure thataccurate, essential, and appropriate written and verbal information is conveyedto the wider MDT to ensure effective management of patients.

Ensuredignity, privacy and cultural and religious beliefs are respected at alltimes.

Adheres toprofessional code of conduct, works within the boundaries of their scope ofpractice and always manage associated clinical risks effectively.

Professional leadership and Management

Actas a role model by demonstrating high standards of holistic care.

Act in such a way as to be a credible,effective leader, demonstrating effective clinical leadership on a daily basis.

Take key responsibility for supporting thewider MDT.

Effectively manage own dairy and workload.

Ensure that documentation is of a very highstandard reflective of Advanced Clinical Practice, adhering to local andnational guidelines.

Identify the skills set and terms ofreference required of the Advanced Clinical Practitioner role, ensuring theyreflect the individual, holistic needs of patients undergoing care.

Works in collaboration with the MDT toinvestigate clinical incidents associated with patients in their care.

Assess and monitor risk in own and otherspractice, acting on results, thereby ensuring safe delivery of care.

Diffuse potential complaints and hostilesituations with staff, patients and carers using highly developed negotiationand interprofessional skills

Provide representation, as appropriate atvarious local and national meetings, providing feedback to the organisation onclinical and professional issues which have an impact on care and standards ofpractice within their sphere of responsibility.

Developformal and informal links outside the organisation, sharing good practice,innovative ideas, and promote staff and service development.

Establishand maintain a regional and national network of contacts relevant to theservice.

Promote theservice through formal and informal presentations within and outside theorganisation through study days, conferences, and written papers.

Education and Training

Hasobtained the theoretical knowledge; MSc in advanced practice or equivalent, inconjunction with practical experience; evidenced in a supporting portfolio, toa level that allows the job responsibilities to be delivered at a highstandard.

Hasobtained Non-Medical Prescribing if appropriate to registered profession.

Identifyown personal developmental and educational needs to work at an advanced leveland beyond. Ensure appropriate action is taken to maintain and further developskills.

Maintain aprofessional portfolio and participate in continuing professional development.

Maintaincompetencies and attend mandatory lectures and training as required by theTrust.

Receiveclinical supervision to clinically improve knowledge and the quality of caredelivered to patients.

Utilisesprofessional knowledge and skills, underpinned by theoretical and relevantpractical experience to teach, motivate, and support junior staff on a range ofclinical practices.

Participatesin the delivery of specialist training and development of other healthcareworkers within the Trust.

Develop andorganise study days and training sessions for self and others within theclinical team.

Participatesin the supervision and mentorship of multi-professional students and juniorstaff.

Undertakeclinical supervision for trainee APs.

Carry outtraining needs analysis, using the results to design, develop and deliver ateaching programme promoting practice development for those staff caring forpatients within the speciality. This will include the development of links withother organisations and innovative approaches to staff development.

Research, servicedevelopment and audit

Maintain upto date knowledge in the specialist field, using information to affect changein practice and ensuring effective dissemination of new knowledge.

Participatein service development. Writes protocols, guidelines, and procedures for ownremit which, impacts on other members of the multi-disciplinary team.

Participatein the development and evaluation of protocols and guidelines and proceduresusing current literature and research.

Participatesin the development of patient information to improve patient care.

Liaise withMDT, including consultants, senior nursing colleagues and management in thedevelopment of services, contribute ideas and make recommendations for serviceimprovements.
