Trainee Advanced Nurse Practitioner (ANP)

Inpatient Unit and Palliative Advice Centre (PACE) at East Cheshire Hospice.

Due to an exciting expansion of services, the hospice is adding to its well-established team.

We are seeking to recruit an experienced nurse with a passion for delivering exemplary care, to join us in supporting people affected by life-limiting illness in both the hospice and community settings.

The successful candidate will be supported to undertake an MSc in Advanced Clinical Practice to facilitate a transition from senior nurse to Advanced Nurse Practitioner.

You will be based in our purpose-built hospice situated one mile from Macclesfield town centre. There is ample free parking available on site.

Our service operates 24 hrs a day, 7 days a week. This post will require weekend and evening work with the possibility of occasional rotation onto nights.

Main duties of the job

Provide high-quality, compassionate care to patients with life-limiting illness

Act as a clinical leader, ensuring safe and effective care delivery

Undertake face-to-face assessments and reviews of palliative patients within the hospice and community settings

Offer advice and support to patients, families, and healthcare professionals both in person and over the phone

Coordinate care and contribute to multidisciplinary team discussions and decision making

Support service development and quality improvement initiatives

Promote evidence-based practice and uphold the highest standards of care

Support the provision of additional hospice services such as blood transfusions and respite care

Job responsibilities JobPurpose

This position has beencreated to facilitate the development of a senior palliative nurse to AdvancedNurse Practitioner. This training will include the undertaking of an MSc inAdvanced Clinical Practice, delivered at a university of mutual choosing, withsupport from the hospice.

The successful candidatewill work alongside the existing nursing and medical teams across all hospiceareas, but primarily within the inpatient unit and palliative advice centre(PACE), being supported by our existing ANPs and medical team to developadvanced clinical skills and prescribing capability.

Within PACE, you will besupported in the delivery of expert palliative advice to community patients,coordinating the delivery of high-quality care- mainly via telephone. The rolefocuses on preventing crises through timely intervention and, where urgentsupport is required, delivering face to face assessment, and supportinggeneralist and junior colleagues.

On the inpatient unit, youwill be a senior clinical presence in the hospice, offering leadership,guidance, clinical advice and assistance. Alongside our existing medical team,you will undertake patient reviews and assessments, liaise with family membersand healthcare professionals and complete internal and external referrals.

Alongside colleagues, youwill also take some responsibility for our respite and blood transfusionpatients and support staff in outpatients when they require senior clinicalinput.

Someteaching/mentorship will also be included in this role.

Duties Clinical Advice & Support
  • You will be providing expertpalliative care advice to patients, families, and healthcare professionals viathe 24/7 Advice Line with support from the wider medical and nursing team whererequired.
  • Where relevant, this adviceshould be in alignment with relevant local and national policies and guidance
  • When required, you will assessand manage urgent clinical situations. This may include the undertaking of homevisits
  • You will deliver hands on careand symptom management for patients in crisis
  • You will make autonomous clinicaldecisions within your scope of competence
  • You will use your clinicalexpertise to assist and advise more junior staff, and participate in wider MDTdiscussions
  • You will help to ensure thedeployment of staff is appropriate for the skill mix and an effective use ofresources
  • Your supernumerary status willallow you to support patients wherever the patient need is greatest, workingboth in the community and inpatient unit
  • PACE is a collaborative effortwith the specialist palliative care team. You will work closely with communityteams, GPs, and other healthcare providers to ensure continuity of care.
  • You will support generalist andjunior colleagues through advice and shared decision making.
  • You will attend MDTs representingPACE and bring relevant patients for discussion these may include localityMDT, GSF, daily Huddles.
Leadership & Professional Presence
  • You will act as a senior nursingpresence during evenings and weekends across hospice services, occasionallyrotating onto nights.
  • You will provide leadership,guidance, and clinical support to nursing teams in all clinical areas.
  • You will contribute to servicedevelopment and quality improvement initiatives within PACE.
  • You will support the on callmanager if required for non clinical issues/incidents out of hours ifnecessary.
Documentation & Compliance
  • You will maintain accurate andtimely clinical records in line with hospice policies and professionalstandards.
  • You will ensure compliance withsafeguarding, infection control, and medication management protocols.
  • Ensure the promotion of thesafety, well being and interests of the patients, staff, and visitors in theclinical area.

  • Observe confidentiality inaccordance with NMC guidance and East Cheshire Hospices policies andprocedures
Control ofInfection

Preventionand management of infection is the responsibility of all members of staff andvolunteers working at East Cheshire Hospice and forms an integral element ofpatient safety programmes. Where control of infection regimes are in force theyare to be always complied with and staff are reminded of the importance ofmaintaining a high standard of personal and environmental hygiene and to followlocal protocols.

  • Motivated and enthusiastic.

Qualifications
  • Registered Nurse with a minimum of 3yrs post qualification experience.
  • Evidence of leadership, management, and speciality development.
  • Relevant additional qualifications in palliative care.

Experience
  • Post qualification experience should include palliative care and nursing in an acute setting
  • Evidence of recent study
