Support Worker - Autism A Chara Disability Services

Contract

Permanent Part time - Flexi part time contract of 48 hours per month minimum.

Responsibilities

• Provide person centred support to individuals with complex needs.
• Plan and deliver support in line with the person's Person Centred Plan (PCP) and care plan.
• Work independently, taking initiative in daily support.
• Contribute to programme planning and support worker role philosophy.

Qualifications & Experience

• Full QQI Level 5 Health Care Qualification (health, community, care sector) or equivalent.
• Good communication skills and fluency in English.
• Desirable: Full, clean driver's licence (manual).

Skills & Abilities

• Ability to work on own initiative.
• Awareness of Key Worker / Support Worker role and philosophy.
• Focus the needs of the person supported as the key focus in all activities.
• Deliver support according to the person's PCP and care plan.
