About the Role
We are looking for a Support Co-ordinator for our PMLD Service, Catherine House. This is a supported living service for 8 young adults with profound and multiple learning disabilities (PMLD) and complex health needs. The service consists of 8 self-contained flats and is located in Hackney. We are looking for an enthusiastic, creative and committed individual to join the management team. If you feel you can bring a fresh approach, some new ideas and a commitment to supporting vulnerable people to have genuine control over their lives, play an active role in the community and develop as individuals then we want to hear from you.
You're Responsibilities
• Provide support to individuals in line with agreed support plans
• Coordinating packages of care and support for individuals and supporting individuals with reviews of support and any needs for long term care or support.
• Demonstrate good IT skills, and be able to record information clearly and professionally
• Good verbal communication skills; be able to liaise professionally with a range of external stakeholders, care managers, health professionals, other services and providers of support, and family members of customers.
• Delivering a service that promotes and supports choice, empowers individuals, helps people
take control of their own support and care, and increases their independence and self-determination.
• Putting the person we support at the heart of your work, promoting each individual's needs for independence, well-being and dignity
• Flexible approach to working shift patterns, which will include evening and weekend hours, and bank holidays.
• Supervision of staff
• Ensuring the service meets regulatory requirements
• Liaising with family and stakeholders
• Working with the deputy and manager to create the rota

- Provide supervision to staff, volunteers and apprentices as requested
- Ensure all staff complete relevant mandatory training within required timeframes
- Provide advice, support and guidance to staff as required
- Undertakes preparation, planning and implementation of work with complex caseloads
- Provides specialist skills, advice, and experience or otherwise to others working with someone with complex needs as appropriate
- To work in a co-operative way at all times with other departments and external agencies
- To act as an ambassador for Outward at all times
- Read and follow the Code of Conduct at all times as well as all policies and procedures
- Take personal responsibility for the safety of self and others at all times
- To work responsibly and appropriately with due regard to confidentiality, commercially sensitive information
- To comply with all legal and regulatory responsibilities as may fall to be applicable
- Take all reasonable measures to follow Health and Safety policies and procedures and appropriate legislation as applicable to the role Uphold the rights of the people we support to be involved at all levels of decision making
- Good knowledge of and understanding of risk and safeguarding
- Good knowledge and understanding of health and safety issues within a supported housing setting
- Experience of providing high quality support
- A Good knowledge of risk assessment and support planning
- Good knowledge of and experience of working within SP and / or CQC standards, the Valuing People framework and the in control agenda
- Good standard of verbal and written communication skills
- Understanding the importance of confidentiality and data protection
- Knowledge of health and safety issues within supported housing or care environments
- Understanding of confidentiality, GDPR, and data protection principle
- Creative thinking and finding practical solutions to problems as well as being results orientated.
- Ability to prioritise work in an environment which may have conflicting pressures and demands, good organisational skills.
- Use of IT skills (Word, Excel and Outlook) to prepare and monitor recording information and reports.
- Ability to deliver flexible person centred services.
- A good level of flexibility and integrity, thinking outside of the box, take accountability, demonstrate passion and positivity Work on own initiative without direct supervision.
- Able to listen and observe, and support emotional needs of a person I Promote the health, safety and wellbeing of young adults at risk and staff

- Staff must have access to a smartphone capable of receiving SMS messages and installing authentication apps for secure systems access

- Maths and English GCSE (or equivalent) Grade C or above
- NVQ Level 2 or 3 in Health and Social Care
- Full UK Driving licence
