The community nursing service within Hackney has recently been restructured with the values of the Homerton at its core. We provide the community nursing service to our patients within the City of London and Borough of Hackney, with the emphasis on delivering the highest standards of care.

We would encourage intuitive, experienced, highly professional community nurses who are motivated to becoming a Specialist Practitioner in District Nursing to apply.

If you are a nurse who is registered on Part 1 of the Nursing & Midwifery Council (NMC) register and are committed to continuous professional development (CPD) we are inviting you to apply for one of our sponsored places that will support you through an academic year of training at the end of which you will join the growing band of Specialist Community District Nurses.

As a qualified nurse, you can apply for a 12 month full-time course or 24 month part-time course. A full-time course comprises of 52 weeks of study, of which 45 weeks are spent in education and clinical practice in London.

The program commences in September 2025 at London South Bank University.

Main duties of the job
  • To develop skills in public health, health needs assessment of practice populations, and health promotion programmes.
  • To develop skills in assessment, planning, providing and evaluating specialist health care related to the needs of individuals, families, groups and communities.
  • Work in partnership with the client groups and other agencies record, implement and evaluate personalised health plans based on an accurate needs assessment, acting at all times in a manner conducive to vulnerable people. Make appropriate referrals and/or mobilise relevant resources, maintaining accurate records at all times.
  • To develop skills and knowledge in identifying and assessing deviations in health and refer promptly to or mobilise appropriate services and resources
  • Work in accordance with the Trust Safeguarding policies and guideline
  • The fundamental responsibilities of the post holder are to attain the academic credentials and the clinical skills required to practice as a District Nurse.
