Pennine Care is committed to improving services for people with Complex Emotional and Relational Needs (CERN) in line with the GM model for treatment of CERN and the National Community Mental Health Framework.
Main duties of the jobYou will work within the Structured Clinical Management (SCM) team, which is part of the Complex Emotional and Relational Needs (CERN) Pathway. In the role of Structured Clinical Management Link Worker you will be providing senior presence and expertise. Your role will be to work with people with CERN who are in the SCM service, and are not yet ready to engage with the SCM programme or therapy, presenting in chaos and with highly risky behaviours. You will act as case holder and actively liaise with other agencies involved to ensure a consistent and multi agency clinically appropriate approach to the person's care. You will have access to SCM training, regular group and individual clinical supervision, and the support of a skilled and friendly team.
The Team is based at Birch Hill Hospital, though we work across the HMR footprint in various community/clinic settings. The Birch Hill site also has some significant benefits, including free and ample onsite parking, excellent canteen, and newly refurbished digital hub which will allow enhanced ICT support and extra space to work and meet with colleagues.
Birch Hill is also a mental health specific site, which allows close working with other secondary care colleagues on the inpatient unit, in EIT and the Complex Emotional and Relational Needs (CERN) team.

- Masters level education or equivalent experience/training.
- Significant evidence of appropriate post-registration training/development.
- Training in Structured Clinical Management.
- Therapy qualification.
- Proven experience of working with people with complex personality disorder.
- Significant post-registration experience working at band 6 level.
- Proven experience of completing mental health assessments, risk assessments and risk management plans without direct supervision.
- Experience of leading MDT and multi agency working coordinated around service users.
- Significant experience of working with service users experiencing suicidal ideation and self-harm.
- Experience of leading multi-agency safeguarding processes/investigations.
- Experience of giving expert advice and supervising the work of professionally qualified staff.
- Previous experience of working to a model of SCM.
- Care coordination of people with personality disorder.
- Completing Care Act assessments and commissioning processes.
You must have appropriate UK professional registration.
