Trinity Hospice is seeking a dedicated Staff Nurse to join our Hospice at Home Night Service, delivering high-quality palliative and end of life care to patients in their own homes throughout the night.
About the Role
As a Staff Nurse in our Hospice at Home Night Service, you'll provide hands on nursing care, symptom management, and emotional support to patients and families. You'll work closely with District Nursing teams and other healthcare professionals to ensure seamless, patient centred care. You'll be a vital part of our night team, helping patients remain comfortable and dignified at home.
Hours per week: 20.

To provide high quality palliative and end of life care to patients in their own homes during night hours ensuring comfort, dignity and support for patients and their families.
Key Responsibilities
Deliver hands on nursing care for patients with palliative needs including physical, psychological and emotional support.
Symptom management for patients nearing end of life including pain control and other complex needs.
Maintain accurate records using EMIS and comply with governance standards.
Liaise with District Nursing teams, FCMS and Inpatient Unit as required for continuity of care.
Provide immediate support to families following death and offer guidance on next steps.
Participate in handovers - teams and telephone and ensure effective communication across services.
Uphold Trinity Hospice values Caring, Adaptable, Responsible, Excellence and Socially Engaged.
Be flexible in working patterns and around rotation to other areas of the hospice to meet the demand of the service.
To act as a role model to others and take charge of the shift in the absence of the senior nurse.
Be responsible for managing the process of receiving and responding to referrals to The Trinity Hospice at home service.
Prioritising workload of the team and managing resource allocation as required.
Act as a mentor and role model for new staff as they undertake their induction programme or students on placement.
Contribute to team meetings and handovers.
Supervise the work of Healthcare Assistants and ensure hospice policies are followed.
Be prepared to cope with incidents or emergency situations as they arise and report them to the Senior Nurse on duty.
To undertake all mandatory training as required by Trinity Palliative Care Services and participate in in service training as and when required.
Maintaining the strict confidentiality of all information acquired especially with regard to patients and staff.
To undertake an appraisal and personal development review annually and through self development continuously update and improve knowledge and competencies.
To take responsibility for being up to date with current policies and procedures and to adhere to these.
Co operating fully in the introduction of any new technology and new methods as appropriate.
To cross work with the District nursing team at night as per memo of understanding between services.
Clinical Review patient symptoms and act accordingly ensuring their symptoms are managed ensuring escalation to seniors if required.
Urethral Catheterisation of patients within the community setting.
Wound care management.
Commence and management of syringe drivers for symptom control.
To contribute to the District Nurses assessments of patients and development or implementation of programme of care for patients in their own homes.
To carry out specific treatments and clinical procedures basing clinical practice on current research based evidence.
To administer medication and assist with their ordering in accordance with NMC standards for the Administration of Medicines.
Perform all duties in accordance with NMC Code of Professional Conduct.
Ensure that relevant health and safety procedures are maintained within patient areas either at their home or within the Hospice.
To establish and maintain effective communications and working relationships with the multi professional team within the community.
To use skilled communication to support carers and families of patients receiving services from Trinity hospice at home service and maintain trusting effective relationships.
To verify when a death has occurred, notifying families and carers as appropriate following the procedures carried out within the community.
To ensure that patients and carers are identified and appropriately referred to other services both from Trinity and community.
To contribute to the development and enhancement of patient care by participating in standard setting policy development and clinical audit of services.
Be responsible for co ordinating flexible care provisions to enable patients to remain at home.
To assist in auditing Trinity hospice at home service with team leadership and participate in research where appropriate.
