What you will do:

• Assess the needs of children with disabilities and their families, ensuring tailored care plans are in place.
• Collaborate closely with multi agency partners to deliver comprehensive support services.
• Implement safeguarding measures and promote the welfare of children and young people.
• Provide expert advice and guidance to families and colleagues regarding disability issues.
• Participate in regular team meetings and contribute to the ongoing development of practice within the team.
• Maintain accurate records and write reports to support key decision making processes.
