Hours: 37 hours
Salary: HC07 (£31,067-£35,235)

The role

We are seeking Senior Social Care Assessors to provide community-based adult social care within Herefordshire. You will mainly work in city settings, applying your knowledge and experience in fast-paced teams. As a Senior Social Care Assessor, you will collaborate with occupational therapists, partners, social care assessors, and social workers. Your scope includes conducting strengths-based assessments, mental capacity assessments, care management, and reviews. A driving license is required as travel is essential for this role.

About You
  • An NVQ3 qualification or above in a related field, or equivalent experience. GCSE or equivalent in English and Maths.
  • Experience working in a care environment.
  • Understanding of current legislation and guidance impacting adult social care.
  • Knowledge of person-centred, strengths-based, outcome-focused assessments, care management, and reviews.
  • Experience supporting individuals to identify and achieve outcomes through strengths-based planning.
  • Ability to develop and maintain good relationships with service users, families, carers, colleagues, and external organizations.
