Senior Pharmacist, Acute Medicine and ED Service

The closing date is 22 October 2025. Interviews are proposed for 12 November 2025. Secondment will be considered for internal candidates. Successful applicants will be required to provide an immunisation and vaccination report from a UK occupational health provider or GP.

Overview

The primary role of this senior post is to lead on the management and provision of a comprehensive, high quality pharmacy service to patients within Emergency and Acute Medicine across the Trust. The post holder will deliver pharmacy related care to these patients Trust wide and support the directorate(s) as needed. This will include providing clinical pharmacy leadership, governance support, education and training, providing expert advice and delivering patient centred care.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job
  • Provide effective clinical and medicines management service to patients admitted and discharged through the Emergency Care Department and Direct Access Pathways of the Trust.
  • Identify areas within Emergency Care and Acute Medicine services where improvements in quality, safety and cost effectiveness of pharmaceutical care and prescribing may be encouraged.
  • Work with senior staff in your areas to review and develop policies and procedures to ensure safe and effective handling of medications.
  • Lead, develop and deliver education and training in the pharmaceutical aspects of Emergency Care and Acute Medicine to other healthcare professionals.

Job responsibilities

The primary role of this post is to lead and provide a comprehensive, high quality clinical pharmacy service to the Emergency Department and Acute Medicine. This includes delivering high quality, cost-effective, safe, efficient and timely pharmacy services for patients and wards within the Trust.

You will be responsible for developing and advising on pharmacy services that affect both the admission and discharge phases of a patient journey, working with teams across the Trust and externally to ensure medications are well managed. This includes screening all TTOs at ward level, supporting the Emergency Department, Medical Admissions Unit, Acute Frailty service and leading/organising a pharmacy team to meet service demands.

You will develop and monitor key performance indicators that are patient outcome oriented.

A significant part of the role is working with Clinical Service Leads, the pharmacy governance team, other clinical pharmacists and the Operational team to ensure a well-led team of pharmacists and technicians providing ward-based, patient-centred care in the ED and Acute Medicine departments.

Part of the role includes mentoring junior pharmacists, developing and delivering training and assessments for rotational pharmacists, Pre-Registration pharmacists, and line management of the Specialist Pharmacist for Acute Medicine. You may also act as Pre-Registration Tutor or clinical coordinator for Diploma Students if required.

In addition to clinical care, you will develop your team and areas to ensure outstanding patient experience.

  • Provide effective clinical and medicines management service to patients admitted and discharged through the Emergency Care Department. Liaise with Pharmacy Managers/Team Leaders, Senior Clinicians and Senior Nurse Managers to coordinate seamless pharmaceutical care and drive service improvements.
  • Proactively influence safe and cost-effective drug therapy with senior clinical staff, including new drug evaluations, financial reporting and policy development.
  • Identify opportunities to improve quality, safety and cost-effectiveness of pharmaceutical care and prescribing.
  • Review and develop policies and procedures to ensure safe handling of medications, ensuring compliance with legal requirements, best practice, guidelines, and risk management; undertake audits related to supply, storage and administration of medicines.
  • Lead, develop and deliver education and training in the pharmaceutical aspects of Emergency Care and Acute Medicine for pharmacy staff and other healthcare professionals, including trainees and diploma students.
  • Encourage and participate in clinical and practice-based research and audits related to drug use in the area.
