Job Description

The Home First service integrates nurses, therapists, health and therapy assistants to collaborate with the acute Multi-Disciplinary Team & Transfer of Care Hubs, promoting efficient hospital discharges to patients' homes.

Our multidisciplinary teams operate flexibly, providing a 7-day service, 365 days a year, from 8am to 8pm. Patients discharged into Home First receive step-down intermediate care, involving community-based assessments and interventions at home to support recovery and monitor health post-discharge, aiming to prevent re-admission. The service also offers acute reablement for up to 10 days to help individuals achieve functional goals and independence in daily activities.

We are seeking a Senior Nurse to lead and develop our team. If you are an ambitious, proactive, and experienced nurse eager to provide excellent community care, we want to hear from you!

Main duties of the job

As a Senior Nurse, you will conduct holistic assessments, develop, implement, and evaluate treatment care plans, and review care provision.

You will manage staff, oversee supervision, and support team development, including shift coordination, visit allocation, and conducting performance reviews.

Your clinical expertise and problem-solving skills will support patient care and team operations, including clinical assessments, medication management, wound and catheter care, and identifying patient deterioration.

You will collaborate with multidisciplinary team members, participate in ongoing learning, and supervise colleagues and students. You will also contribute to service development through reflective practice, clinical supervision, and quality improvement initiatives, with opportunities to develop leadership skills through formal training.
