We're looking for individuals who want to grow and develop their knowledge and skills by working closely with colleagues, sharing experience and knowledge to continually learn from each other.
Main duties of the jobOlder peoples nursing can be overlooked, however the complexity of the skills required for care for a patient with dementia embodies what mental health care is about and pulls on every facet of our training and knowledge base.
You will be become an essential part of our multi-disciplinary team of Consultants, Speciality Doctors, Psychologists, Community Matron, Clinical Team Manager, Advanced Nurse Practitioner, Non-Medical Prescribers, Nurses, Assistant Practitioner, Care Support Workers and Occupational Therapists all supported by our fantastic admin team.
As a team we feel it is vital to raise awareness around the need for more research, early intervention and sign posting, and you will play a vital role in this, using your existing expertise and supported with continuous learning from the supportive team.
This diverse range of specialities in the team allows us to work collaboratively with the aim of providing high quality and integrated care.
Please note that this is a community based role, the successful candidate will need to have a full UK driving licence and access to their own car.

Person Specification Qualifications- Registered Mental Health or Learning Disability Nurse, Occupational Therapist or Social Worker with the relevant registration
- Practice Assessor and Practice Supervisor training (NMC registrants) or Practice Educator (AHP & Social Worker Registrants
- Ongoing professional development working towards degree level (if professional registration is following a diploma level course)
- Recognised Leadership / management training
- Higher degree or post graduate certificate in area of clinical / therapeutic practice
- Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
- Demonstrate effectice risk assessment and risk management experience
- Demonstrable experience in assessment of mental health
- Excellent communication and engagement skills
- Competent assessment skills and effective analytical and decision making skills
- Care planning, risk assessment knowledge and skills
- The ability to use clinical assessment tools and outcome measure effectively and collaboratively
- An understanding of the policies and guidelines both local and national
- Up to date knowledge of legal and ethical frameworks including Mental Health Act
- Ability and willingness to travel independently to meet the needs and requirements of the service
