Senior Grade Occupational Therapist - Dublin

Cpl Healthcare, in partnership with the HSE, is seeking a Senior Clinician / Clinical Coordinator to support service delivery within the Chronic Disease Hub, based at the Bray Integrated Care Hub, Civic Centre, Bray, Co. Wicklow.

Role Details:

• 1.0 WTE Senior Clinician / Clinical Coordinator
• Senior clinical professional with appropriate registration and experience in chronic disease management
• Experience working within integrated care, primary care, community services, or chronic disease programmes is desirable
• Experience in operational coordination, multidisciplinary team working, patient pathway management, and service development is preferred
• Knowledge of the Integrated Care Programme for Chronic Disease and community-based specialist services would be advantageous
• The post will support the coordination and delivery of Chronic Disease Hub services, working closely with medical, nursing, health and social care professionals, administration, and management colleagues
• The role will provide operational leadership, support service planning and patient flow, waitlist management and oversight, operational management of administrative team and contribute to the ongoing development of integrated care pathways
• Contract duration: 3 months initially
• Location: Bray Integrated Care Hub, Civic Centre, Bray, Co. Wicklow
