Senior Clinical Pharmacist for Medicine

To lead, develop and participate in specialist clinical pharmacy services within the medical business unit, providing expert advice on medicines management to ensure safe, effective and high quality care to patients.
Main duties of the job- To provide specialist clinical pharmacy input and maintain high standards of medicines management across the medical business unit.
- To work closely as part of the MDT to develop high standards of prescribing, clinical reviews and a focus on polypharmacy and de prescribing.
- As part of the team you will work closely alongside other pharmacy colleagues, providing leadership, clinical supervision and management.
- Contribute to horizon scanning in the pharmacy department to look for opportunities to promote medicines optimisation and improve transfer of care between hospital and community.
- Work to deliver the 7 day clinical Pharmacy service by supporting the late night, on call and weekend rota.
- Provide support to the Lead Clinical Pharmacist with regards to medicines management, incident resolving, staff training and integration with the medical wards.
- Engagement with key stakeholders e.g ward managers, matrons, chief matron, service line managers and clinical and safecare leads.

Job responsibilitiesThe Senior Clinical Pharmacist for Medicine will be responsible for the delivery of clinical pharmacy services and medicines management within the Med 2 directorate. This includes the cardiology, diabetes and endocrinology, respiratory, gastroenterology and general medical wards. Advanced clinical pharmacy knowledge and independent prescribing qualification will support patients throughout their hospital journey.
The role will involve direct line management and leadership of pharmacy staff including appraisals, objective setting and providing support and professional development opportunities to the team.
Updates will be provided to the Lead Clinical Pharmacist for Medicine to ensure continuous review of clinical services provided and monitoring of key performance indicators.
MDT working closely with senior leaders within the medical business unit including senior medical and nursing staff, matrons and service line managers to support service delivery, incident reviews and medicines governance.
Maintain and evidence own clinical and professional development and support the education, training and supervision of others within the team and the medical directorate.
Work to deliver the 7 day clinical Pharmacy service by supporting the late night, on call and weekend rota.
Person Specification Experience- Clinical pharmacy experience in a hospital setting
- Experience of leadership
- Experience of clinical audit or quality improvement
- Experience a line manager
- Experience of service development or improvement
- Evidence of in depth clinical pharmacy knowledge
- Excellent written and verbal communication skills
- Specialist knowledge of Pharmacy systems
- Evidence of self-reflection and continuous professional supervision
- IT literate
- Advanced clinical skills
- Evidence of innovative practice
- Registered as a pharmacist with General Pharmaceutical Council (GPhC)
- Diploma in Clinical Pharmacy or equivalent experience
