About this job

Ensure the development and articulation of best practice within the service area and contribute across the service by exercising the skills of a reflective practitioner, taking part in regular professional supervision and appraisal and maintaining an active engagement with current developments in the field of Advanced Practice.

Collaboratively work with MDT colleagues across the organisation and beyond to streamline pathways to support admission avoidance and facilitate discharge.

Exhibit the highest level of interpersonal and communication skills and develop effective communication strategies with other members of the multidisciplinary team The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £54.
