Main duties of the job

In accordance with the practice timetable, as agreed, thepost-holder will make him/her-self available to undertake a variety of dutiesincluding surgery consultations, telephone consultations and queries, visitingpatients at home, checking and signing repeat prescriptions and dealing withqueries, paperwork and correspondence in a timely fashion. This will includeinsurance reports, DWP reports, and other correspondence required by patientseither for NHS patients care or in connection with medical/social careinsurance purposes.

The practice has a great team, which includes 4 GP Partners, 1Salaried GP, Nurse Consultant, Urgent Care and Nurse Practitioners, ClinicalPharmacist, Practice nurses and a Nurse Associate all supported by strong adminand reception teams. We have recently achieved high QOF figures, our last CQCinspection rating was Good. We are also in a very active Primary Care Network(Wrekin PCN) that currently has the following staff: PCN Physio, PCNPharmacists and Pharmacy Technician. The Practice has an active and supportivePatient Group.

Job responsibilities

This will includeinsurance reports, DWP reports, and other correspondence required by patientseither for NHS patients care or in connection with medical/social careinsurance purposes.

Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other health careworkers within the organisation

Assessing the health care needs of patients withundifferentiated and undiagnosed problems

Screening patients for disease risk factors and earlysigns of illness

In consultation with patients and in line with currentpractice disease management protocols, developing care plans for health

Providing counselling and health education

Admitting or discharging patients to and from the caseloadand referring to other care providers as appropriate

Recording clear and contemporaneous consultation notes toagreed standards

Collecting data for audit purposes, significant events,including QOF data and other data required by the practice.

Compiling and issuing computer-generated acute and repeatprescriptions (avoiding hand-written prescriptions whenever possible)

Prescribing in accordance with the practice prescribingformulary (or generically) whenever this is clinically appropriate

In general the post-holder will be expected to undertake allthe normal duties and responsibilities associated with a GP working withinprimary care.

Person Specification Qualifications

• You must be a fully qualified GP on the GP Performers List
• Working knowledge of clinical system (System One, EMIS)
• Excellent written and verbal communication skills and time management
• Current valid driving licence/access to a car.
• Previous general practice experience is desirable.
• completion of clinical audits, working in a team, general understanding of the GMS contract and QOF.
• A good understanding of patient choice and referral system.
• Pleasant and supportive personality.
• Ability to perform well under pressure.
• Resourceful, flexible & adaptable approach to work
