Purpose of the Post

The RANP Older Persons Rehabilitation will practice at a high level of capability as an independent, autonomous and expert practitioner. The overall purpose of the RANP Older Persons Rehabilitation service is to provide safe, timely, evidence based nurse led care to Older People at an advanced nursing level. This involves undertaking and documenting complete episodes of patient care, which includes comprehensively assessing, diagnosing, planning, treating and discharging patients in accordance with collaboratively agreed local policies, procedures, protocols and guidelines and/or service level agreements/ memoranda of understanding. The RANP Older Persons Rehabilitation demonstrates advanced clinical and theoretical knowledge, critical thinking, clinical leadership and complex decision making abilities. The RANP Older Persons Rehabilitation practices in accordance with the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (NMBI 2014), Scope of Nursing and Midwifery Practice Framework (NMBI 2015), Advanced Practice (Nursing) Standards and Requirements (NMBI 2017), and the Values for Nurses and Midwives in Ireland (Department of Health 2016). The RANP Older Persons Rehabilitation service provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence based health policy at local, regional and national levels. The RANP Older Persons contributes to nursing research that shapes and advances nursing practice, education and health care policy at local, national and international levels. The RANP Older Persons Rehabilitation will address unmet needs with older persons with frailty by supporting the residential care services on campus.

Main objectives of this RANP Older Persons Rehabilitation post at Peamount Healthcare
  • To lead on the development of the Advanced Practice Older Persons nursing service at Peamount Healthcare in collaboration with nursing, medical and health and social care professional colleagues.
  • To manage an autonomous caseload of patients identified collaboratively within an agreed scope of practice (Domain 1).
  • To expertly assess the older adult, providing specialist geriatric opinion through holistic Cognitive Geriatric assessment (CGA) of the person including treatment and diagnosis at advanced level and refer to other health professionals as appropriate.
  • Accurately documenting assessment, diagnosis and integrated plan of care.
  • To combine skills and knowledge on a range of social, biological, and psychological variables in the provision of holistic care.
Point on Salary Scale
  • New appointees coming from the private sector will be placed on the minimum point of the scale
  • Clinical and Therapy Grades and Staff Nurses: Incremental credit may be applied for recognised relevant service in Ireland and abroad (Department of Health Circular 2/2011) in the Civil Service, local authorities, public service, health service and other public service bodies and statutory agencies.
