Reablement Support Worker

£12.21 - £12.45 per hour. Mileage paid at £0.45 per mile.

Firstcare Staffing Solutions are recruiting agency Reablement Support Workers for our client to cover a variety of reablement shifts across Bedford Borough.

We are looking for individuals that can work a minimum of three days/twenty hours per week. Shift patterns are 7.30am to 2.30pm and 2.30pm to 10.00pm. The shifts are flexible between Monday to Sunday and you will be able to pick your own shifts to suit your lifestyle.

To apply for this role you must hold a valid driving license and have access to a vehicle. You will have at least six months UK care experience, ideally in a community setting.

Job Purpose

To provide support, encouragement and physical assistance (where required) to adults in their own home to increase confidence and independence.

To assist with the facilitation of discharges from hospital and respite placement to ensure a timely and safe transfer to the community.

To assist with the prevention of admission to hospital to hospital by providing skilled level of support under guidance of Coordinator and/or Team Manager.

To ensure that all clients have an outcome focused support plan, which is updated as their independence increases.

Main Duties

To contribute to the continual monitoring/reviewing of the service users progress within the Individual Support Plan and accurately communicate any changes to the Reablement Coordinator and/or other social care professionals. To complete Customer Measurement tool outcomes.

To build up positive working relationships with the service users and their family/informal carers, using varies communication and enabling skills according to the individual situation.

To identify areas of need where service users may require practical assistance to ensure safety and/or regain independence.

To identify when equipment may be required to promote safety and independence and action accordingly.

To keep accurate and timely records in accordance with departmental policy, ensuring that appropriate terminology is used and satisfying legal requirements.

To attend briefings and team meetings

To appropriately deal with emergency situations with service, which may arise during the course of the work, under guidance of Reablement Coordinators/Team Managers/Other professionals.

To identify alternative forms of support for individual service users and action requests as appropriate.

To attend reviews, case conferences and professional meetings as required.
