The Community Resource team Home Care (CRT) are part of Cardiff Council's Independent Living and Community Care Service (ILS) and work in partnership with vulnerable adults to support them to live independently at home and connected to their communities. CRT is a joint service provided by Cardiff Council and Cardiff and Vale University health board that aims to support adults, through therapy and/or home care, to recover or maintain their ability to live independently at home. The service provides reablement care which aims to encourage and support people to learn or re-learn skills necessary for daily living, following a period of illness or after a stay in hospital.
What we are looking for from youWe are looking for those with proven experience of Line management in a regulated service and working with vulnerable adults in their own homes. You will need to demonstrate your ability to work to a preventative model focusing on strengths and outcomes to promote independence along with problem solving and decision making skills. You will have a QCF Level 4 or above in Health and Social Care or equivalent professional qualification. You will have experience of working and communicating effectively with partner agencies in a range of departments within a local authority, health and third sector organisations. You will need the ability to extract, analyse and report data to support the running of an effective and well managed service. The ability to speak other languages, in particular Welsh, and Community Languages would be an advantage.
Responsibilities and duties• Line management of a cohort of officers within CRT Home Care, in accordance with all regulations related to a Domiciliary Social Care services, including: the Social Care and Wellbeing Act 2014, RISCA 2016, Cardiff Council policies, procedures, and all statutory duties.
• Support the Senior Management Team and utilise services from internal and external partners to develop effective pathways for hospital discharge reablement care needs or to avoid hospital admissions.
• Work across Health, Social Care, third sector and Local Authority to strengthen partnership arrangements and deliver corporate objectives to support service users with reablement potential and independent living at home.
