Advanced Practitioner - Community Mental Health Team
Location: North West
Paying £32 per hour
We are seeking an experienced and passionate Social Work Advanced Practitioner to join the Community Mental Health Team. The ideal candidate will have 2-3 years' experience within Adults Social Care. You will uphold and champion best practices, focusing on proactive problem-solving to meet the needs of vulnerable adults, reducing the risk of harm. The role involves a mix of office-based work and visiting clients in hospitals and their own homes. Mental health experience, report writing, attending Mental Health Tribunals, and familiarity with WCCIS are essential, especially given the short contract duration. Local knowledge or willingness to travel is preferred.
In this role, you will work in accordance with social care legislation and local government requirements, supporting the delivery of person-centered, safe, and effective social care services. Using your professional knowledge and experience, you will make assessments and decisions on appropriate interventions. You will establish and maintain relationships with service users and other agencies to ensure services meet identified needs.
You will be committed to achieving positive and sustainable outcomes through your interventions, striving to provide the best results for children and families, with a focus on protection and efficiency.
Perfect candidate will be:
- Qualified Social Worker registered with Social Work England
- Experienced in working with children and families, including vulnerable groups
- Experience with Court, Children in Need, and Child Protection
- Supporting child and family cases involving CIN and Child Protection planning, pre-proceedings, and court proceedings
- Understanding factors impacting children, including abuse and neglect
- Ability to undertake Child & Family Assessments with effective analysis, decision-making, planning, and review
- Capable of direct work with children and families
- Creative, flexible, and forward-thinking approach
- Knowledge of Child Protection legislation and issues
- Committed to continuous improvement for the best outcomes
- Excellent mediation and negotiation skills
- Strong relationship-building skills with children and families
- Experience working in a multidisciplinary, interagency environment
- At least 2 years post-qualification experience

- Manage an agreed caseload
- Use specialist communication skills for optimal outcomes
- Develop individual care plans respecting diversity, dignity, and empowerment
