Job Description

Newlands Medical Centre is looking for a prescribingclinical pharmacist to join our highly skilled and experienced team.

The post requires autonomous working to providepharmaceutical care to our patients. You will have daily appointments to offermedication reviews and medication advice as well as minor alignments and othercontractual obligations including QOF and IIF schemes.

Please note this role is an ARRS funded role through Greater Middlesbrough PCN, based fully at Newlands Medical Centre.

Main duties of the job

The role offers a varied work agenda providing medicinesoptimisation support and expert pharmaceutical advice, including theimplementation and delivery of prescribing initiatives, improving prescribingsafety and provision of medication reviews, fully supported by an experiencednetwork of colleagues. The successful applicant should be able to demonstrateexcellent clinical, and communication skills and have experience ofimplementing medicines optimisation, quality and patient safety initiatives andresolving prescribing issues. You will be a Registered Pharmacist with theGeneral Pharmaceutical Council, who is an Independent Prescriber. You must alsobe computer literate, organised and be able to work independently as well aspart of a team. The successful candidate will be able to prioritise workload,build key relationships and be confident interacting with a wide range ofstakeholders such as GPs, Nurses, secondary care clinicians and patients.

We have Investors in People Gold Award and are committed to developing our team to provide quality care to our circa 10,000 patients. We are a training practice and teach GP Registrars. We are a member of Greater Middlesbrough PCN and work closely with our other 7 member practices.

Our Primary Care multidisciplinary team currently consists of 4 GP partners, 4 salaried GPs, 3 practice nurses, 1 nurse associate, 2 health care assistants, clinical pharmacist, and pharmacy technician. We also have allocated ARRS roles via our PCN which included an FCP, a Dietician, social prescribers, mental health workers. Attached staff include midwife, health visitors and district nursing team.

Job responsibilities
  • Main priority of the role will be to support with medicines-related activity such as completing medication reviews, SMR's, carrying out QOF work, medicines optimisation, and CQC audits.
  • The post requires autonomous working to provide pharmaceutical care to our patients. You will have daily appointments to offer medication reviews and medication advice as well as minor alignments and other contractual obligations including QOF and IIF schemes
  • Completing medication reconciliations and reviews for patients SMR both in person and over the telephone
  • Complete SMR
  • Medicine optimisations and medicine prescription.
  • Carry out benzodiazepine reviews.
  • Prescribing medication.
  • Must be able to offer sound and professional advice in relation to clinical management issues
  • Maintain and improve quality and patient safety
  • Provide pharmaceutical input for clinical governance meetings
  • Participate in relevant network and peer review meetings as required.
  • Work with community pharmacies locally to ensure alignment with delivery of both the new Medicines Service (to support adherence to newly prescribed medicines) and supporting the development of medicines reconciliation service (to support effective transfers of care between hospital and community).
  • Commit to ensuring own continued professional development, working alongside Senior Clinical Pharmacist and Clinical Leads to determine key development areas relevant to the post-holder and the emerging needs of the PCN
  • Ensure awareness of current educational policies relevant to working area of practice and keep up to date with relevant clinical practice.
  • Providing brief intervention and raising awareness to support key public health campaigns, including supporting early cancer diagnosis and lifestyle advice/support.
  • Provide specific medicines optimisation support and lifestyle advice to patients at high-risk of development CVD, including initiation of statins, anti-hypertensives and dose optimisation.
  • Offering anticoagulation to patients with atrial fibrillation who are not on treatment despite no contraindications. Providing medicines optimisation advice and support for patients with heart failure.
Person Specification Experience
  • Experience of undertaking medication or medicine use reviews, and patient counselling.
  • Experience of working with clinicians and multidisciplinary/multi-agency work.
  • Experience in undertaking clinical audits.
Qualifications
  • Degree in pharmacy or equivalent.
  • Registered with GPhC.
  • Evidence of recent CPD
  • Specialist knowledge through a Postgraduate Diploma & Qualification.
  • At least 2 years post registration experience in a hospital, community or general practice setting.
