Prescribing Clinical Pharmacist

We are looking for somebody who shares our idea ofwhat a PCN should be - supportive of its member Practices, effective and innovative.Youll be prepared to work hard to look after your patients and work with the leadclinical pharmacists, wider multidisciplinary team colleagues and PCN leadershipteam to maintain a resilient and sustainable PCN.

Main duties of the job We are looking for a prescribing clinicalpharmacist to join our team who acts within their professional boundaries, to work alongside a team ofpharmacists in our PCN which is made up of 5 Practices.

We're seeking someone who wants to improve patients health outcomes and the efficiency of the primarycare team by providing direct, accessible andtimely medicines and expertise including face to face contact with patients.

You will be anintegral part of the PCN, as well as part of a wider general practice andmulti-disciplinary team. This includes being a conduit of patient medicinesinformation into and out of hospitals. You will be asked to work to optimise medicationissues to improve patient care and safety,and augment clinical staffin the management of patients, with minimal support needed from Senioror Lead Pharmacists.

You must have completed CPPE pathway and must be an IndependentPrescriber.

Supervision and a mentorship networkwill be providedto support the prescribing clinical pharmacist.

Job responsibilities Primary responsibilities:

A prescribing clinical pharmacist at Network North PCN has the following key responsibilities in relation to delivering health services.

There will, on occasion, be a requirement to carry out other tasks;this will be dependentupon factors such as workload and staffing levels.

a.Work as partof a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge ofmedicines for specific disease areas.

b.Be a prescriber and work withand alongside the PCN and general practice team

c.Work with the 5 Practices across the PCN as and where needed

d.Be responsible for the care management of patientswith chronic diseases and undertake clinical medication reviews toproactively manage people with complex polypharmacy, especially the elderly, people in care homes, thosewith multiple co- morbidities (in particular frailty,COPD and asthma) and people with learning disabilities or autism (throughSTOMP Stop Over Medication Programme)

e.Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the organisation and to help in tackling inequalities. Comparedto a Clinical Pharmacist role, they must have a greater range of clinical skillsrelating to competences

f.Provide leadership on person-centered medicines optimisation (including ensuring prescribers in the PCN practicesconserve antibiotics in line with local antimicrobial stewardship guidance)and quality improvement, whilst contributing to the Quality and OutcomesFramework and enhanced services

g.Through structured medication reviews, support patients to take theirmedications to get the best from them, reduce waste and promoteself-care

h.Have a role in supporting further integration ofgeneral practice with the wider healthcareteams (including community and hospital pharmacy) to help improve patient outcomes, ensure betteraccess to healthcare and help managegeneral practice workload

i.Develop relationships andwork closely withother pharmacy professionals across the wider health and social care system

j.Take a central role in the clinical aspectsof shared care protocols, clinicalresearch with medicines, liaison with specialist pharmacists (includingmental health and reduction of inappropriate antipsychotic use in people withlearning difficulties), liaison with community pharmacists, and anticoagulation

k.Bepart of a professional clinicalnetwork and have access to appropriate clinical supervision.

l.To act as the point of contactfor all medicinerelated matters, establishing positive working relationships

m. To consultpatients within definedlevels of competence and independentlyprescribe acute and repeat medication

n.To receive referrals and directed patientsfrom triage servicesand other clinicians

o.To receive and resolve medicines queries from patientsand other staff

p.To provide medication review services for patients in the practices and duringdomiciliary visits to local nursing homes

q.To manage a caseload of complex patients

r.To manage a therapeutic drug monitoring system and the recall of patients taking high risk drugs, i.e.,anticoagulants, anticonvulsants and DMARDs, etc.

s. To deliverlong term conditionclinics and home visits, particularly for patients with complicated medication regimes, andprescribe accordingly

t.To provide pharmaceutical consultations to patientswith long term conditions as anintegral part of the multidisciplinary team

u.To review medications for newly registered patients

v. To improvepatient and carerunderstanding of confidence in and compliance with their medication

w.To maintain accurateclinical records in conjunction with extant legislation

x. To encourage cost-effective prescribing throughout the PCN

y.To implement and embed a robust repeatprescribing system

z. To provideadvice and answermedication related queriesfrom patients and staff

aa.To organise and oversee the PCNs medicines optimisation systems, including the repeat prescribing andmedication review systems

bb.To improve the quality and effectiveness of prescribing throughclinical audit and education, to improve performanceagainst NICE standards and clinical and prescribing guidance.

cc.To develop yourselfand the role through participation in clinical supervision, training and service redesignactivities

dd.To ensure appropriate supervision of safe storage,rotation and disposal of vaccines and drugs. To apply infection-control measureswithin the PCN practices according to local andnational guidelines

ee.To provide subjectmatter expertise on medication monitoring, implementing andembedding a robust system

ff.Share expertise with the variousstudents across the Practices and have students shadow them in clinic

gg.To support clinicians with the management of patients suffering from drug and alcohol dependencies

hh.To actively signpostpatients to the correct healthcare professional

ii. To manage a caseload ofcomplex patients and potential care institutions and to provide advicefor the GP management of more complexpatients or areas such as addictive behaviours, severe mentalillness or end of life care

jj.To review the latest guidance, ensuring the organisation conforms to NICE, CQCguidance etc.

kk. To providetargeted support and proactive reviewsfor vulnerable, complexpatients and those at risk of admission and re-admission to secondarycare

ll.To handle prescription queries and requestsdirectly

mm.To provide proactiveleadership on medicinesand prescribing systemsto the organisation, patients and their carers

nn. To supportin the delivery of enhancedservices and other service requirements on behalf of the organisation

oo. To participate in the management of patient complaints when requested to do so, and participate in the identificationof any necessary learning brought about through clinical incidents andnear-miss events

pp.To undertake all mandatory trainingand induction programmes

qq. To contribute to and embracethe spectrum of clinical governance

rr. To attend aformal appraisal with your manager at least every 12 months. Once aperformance/training objective has been set, progress will be reviewedon a regular basis so that new objectives can be agreed

ss. To attend pharmacyteam meetings and other clinical team meetings

tt. To contribute to public healthcampaigns (e.g., COVID-19or flu clinics) throughadvice or direct care

vv. Support deliveryof QOF, IIF, other incentive schemes, QIPP and other qualityor cost effectiveness initiatives

Secondary Responsibilities:

In addition to the primaryresponsibilities, theprescribing clinical pharmacist may be requestedto:

a.Agree and review prescribing formularies and protocols, and monitor compliance levels

b.Improve the data quality of medicines recordsand linking to conditions

c.Deliver training, mentoring and guidance to other clinicians and staff on medicineissues
