Prescribing Adviser & Medicines Formulary Manager (XR09) Thisis a fantastic opportunity for an experienced and enthusiastic pharmacistlooking to further their career, leadership and clinical skills within theLeeds Medicines Advisory Service at the Leeds Teaching Hospitals NHS Trust(LTHT).

Ourwell-regarded Leeds Medicines Advisory Service incorporates the MedicinesAdvice Service (including our busy query answering service), medicines safety,formulary, medicines commissioning and primary care collaboration. We supportand promote evidence-based decision-making about medicines, and contribute tothe safe and effective use of medicines within LTHT and across the interfacewith primary care.

Main duties of the job Thepost holder will play a key leadership role in the delivery, management anddevelopment of a highly specialised pharmacy service across the Trust, workingclosely at the interface between secondary and primary care. The role involvesleading and directing the provision of expert prescribing advice andcontributing to medicines formulary management across the primarysecondarycare pathway. The post holder will ensure that medicines are usedappropriately, safely and cost-effectively, in line with Trust policies,standard operating procedures, commissioned arrangements and medicineslegislation.

Applicantsshould have experience working across the interface and have extensiveexperience in management and clinical pharmacy, as well as experience with Drug& Therapeutics and medicines formulary processes. You should have knowledgeof prescribing practice recommendations and guidance. You will be supported byan experienced team.

Thepost-holder will also deliver a high quality clinical pharmacy service toallocated wards/teams and manage service provision by other pharmacy staff.

Job responsibilities JOBPURPOSE/SUMMARY

Tomanage and develop a highly specialised element of the pharmacy service acrossthe Trust and at the interface between the Trust and primary care.

Providespecialist pharmaceutical professional leadership for a defined area ofPharmacy services

Provideexpert opinion, both within and outside pharmacy to ensure the safe andeffective delivery of medicine therapy

Toensure medicines are used appropriately, safely and cost-effectively inaccordance with Trust policy, standard operating procedures, commissionedpositions and medicines legislation. Thepost-holder is expected to determine how the outcomes are best achieved inspecific circumstances

Beresponsible for analysing and reviewing specialist information to developpolicy and procedure for the defined area of responsibility

Managea team of specialist pharmacists, or the provision of a specialist pharmacyservice eg prescribing advice

Responsiblefor the provision of information to Trust & Primary care committees andClinical Service Units (CSU) regarding clinical and financial governance withintheir speciality, including issues around service delivery and futuredevelopments

Responsiblefor the co-ordination of training for pharmacists and other healthcareprofessionals on aspects of their specialities incorporating training onadvanced skill development e.g. formulary management, risk assessment, researchprotocol development

Providesessional cover in an operational service area and/or specialist clinicalpharmacy provision to patients. Participates in the MMPS rota that providesservices and activities across a 7-day week

PRINCIPALDUTIES & AREAS OF RESPONSIBILITY

Leadand direct the provision of a highly specialised element of pharmacy services-prescribing advice and medicines formulary management across theprimary/secondary care interface.

Toensure robust policies and procedures are in place to effectively managemedicines

Tobe responsible for development of service and policy implementation

Beresponsible for individual and team performance through the setting ofobjectives, PDPs and appraisals. Also be responsible for recruitment strategiesincluding undertaking skill mix reviews and succession planning.

Provideexpert opinion, both within and outside pharmacy, to ensure the safe andeffective delivery of medicines therapy and in relation to area of expertise.

Toact as a role model and identify opportunities to provide leadership acrossMMPS CSU and more widely across the Trust.

Providepharmaceutical professional leadership for a defined clinical area or serviceby liaison with all relevant pharmacy stakeholders and key members of anyrelevant CSUs.

Beresponsible for ensuring that the legal and statutory requirements in relationto medicines and the specialist service elements are met and ensure that thedelivery of pharmaceutical care to patients and customers in the defined areasof use meets both national and local standards

Whereappropriate, lead the highly specialised pharmacy service to patients and themultidisciplinary team within a given clinical speciality.

Demonstratethe ability to provide safe, clinically effective and cost-effective use ofmedicines in accordance with Trust policy, standard operating procedures,commissioned treatment pathways and medicine legislation. This will includeprescription review, advising on dosage and side effects and on safeadministration of medicines. In specialist services this may occur for a groupof defined patients as well as individuals. The post holder is expected todetermine how the outcomes are best achieved in a specific circumstance.

Ensurethat funding is secured for service developments through business case andnegotiation with stakeholders outside pharmacy

Analyseand provide detailed information regarding medicine use and financial data forstakeholders to influence external decision making.

Toprovide medicine forecasting information and advice to CSUs or Trust committeeson options to allow effective budget planning.

Bean expert member of Trust and primary care medicines optimisation committeesrelating to the use of medicines and their place in commissioned pathways withthe aim of providing expert advice.

Trainpharmacy staff to an appropriate level of competence depending on their role.The post holder, will participate in the training of other healthcareprofessionals, within the Trust and Academic partner institutions, inspecialist pharmacy service related subjects.

Promoteresearch activities of the individual and team, by identifying areas for auditand ensuring support from CSUs or Trust. The post holder will undertake andpublish practice research and enable team members to do likewise.

Thepost holder will be committed to the concepts of continuing professionaldevelopment and lifelong learning.

Workagreed sessions in a dispensary area and providing a full range of dispensingservices.

Contributeto providing roles across 7 days and non-core hours as determined by agreedrotas.

Qualifications
  • MPharm, MPharmS, or MSc Pharmacy degree and GPhC registered pharmacist
  • Post-graduate MSc/Diploma in Clinical Pharmacy and/or information science
  • Postgraduate management or leadership qualification
Other criteria
  • Organisational ability, excellent oral and written communication skills, negotiation skills and decisions making skills
  • Confidence to speak at meetings
Experience
  • General knowledge of NHS developments including primary and secondary care
  • Significant experience of working in a hospital pharmacy with some time in a managerial role.
  • Evidence of planning and development successes and involvement in service developments
