PCN Mental Health Practitioner

An exciting opportunity has arisen for dedicated and enthusiastic Mental Health Practitioners to join our growing team of multidisciplinary clinicians providing care to patients within the Primary Care Network footprints of Nottingham and Nottinghamshire. These pivotal roles work across primary and specialist services within an integrated community mental health model.

In Nottingham and Nottinghamshire there are 23 PCNs with a variety of sizes, patient populations and mental health priorities. During 2021, each PCN will have at least one mental health practitioner providing a named clinician for the GP practices within that locality.

The posts will be co located within Hucknall's Byron PCN and be responsible for providing specialist mental health care and interventions to patients within the PCN using high level assessment to understand needs, treatment options, refer appropriately and coordinate the patient's journey across health, social care and voluntary sector systems, regardless of care setting. It is essential that these posts forge close working relationships with professionals both within the PCN and local Mental Health Trust to enable comprehensive and patient centred care plans to be developed and delivered.

Main duties of the job

The post holder will:

  • Provide advice, enhanced assessment, support and treatment for those experiencing problems with their mental health and emotional wellbeing in primary care.
  • Provide support to the all age adult mental health model.
  • Provide clinical advice and support to non mental health clinicians in a PCN.
  • Be part of the interface between primary care and specialist mental health services in the Locality Team.
Job responsibilities

The role will be hosted by the Broxtowe & Hucknall Local Mental Health Team under Adult Mental Health Services but work within the Primary Care Network.

This unique opportunity to work across both primary care and specialist mental health services will support the development of new ways of working to support service users and their families.

Requirements for applying

To apply for these roles you will need:

  • Significant post qualification experience within the management of patients with complex needs.
  • Excellent communication and liaison skills with the ability to work autonomously.
  • Primary care experience is not essential but an understanding of how PCNs work would be beneficial.
  • Motivation and flexibility to develop the service to meet the needs of the PCN population.

In addition, the post holder will need to have excellent managerial and leadership skills and will be expected to support service transformation by leading on service change to embed new ways of working across primary and secondary care in line with the Community Mental Health Framework for Adults and Older Adults (Long Term Plan).

A full UK driving licence and vehicle for business use is required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced); this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. Person Specification Qualifications
  • Professional Qualification (RMN/RNLD, Social Work, OT) to degree level or equivalent.
  • Further specialist knowledge/post registration experience in a specific service area or other formal training.
  • Maintain current registration with the appropriate body (NMC/HCPC).
  • Practice based educator qualification or equivalent (or prepared to undertake training).
  • Demonstrate continued professional development to maintain professional registration.
Training
  • Willingness to undertake new clinical practices, education and training that will enhance service delivery.
Experience
  • Demonstrates satisfactory clinical expertise for the banding.
  • Specialist and area specific experience.
  • Effective risk assessment and risk management experience.
  • Experience in the delivery of clinical interventions.
  • Experience of working autonomously within a senior position.
  • Experience of managing staff and good leadership skills.
  • Experience in supporting service development.
  • Ability to deal with and resolve conflict.
  • Experience of leading training sessions.
  • Experience of leading service change/development/transformation.
Knowledge
  • Knowledge and understanding of current legislation and clinical risk assessments.
  • Good knowledge of the national agenda for mental health.
  • Knowledge of local wider systems and networks.
  • Knowledge of health promotion.
  • Awareness of national and local issues impacting healthcare.
  • Knowledge of management theories.
Skills
  • Ability to work as part of a team.
  • Flexibility and ability to handle conflict issues.
  • Good skills in assessment, intervention and evaluation of patient care.
  • Clinical supervision skills.
  • High level of emotional intelligence.
