LeedsStudent Medical Practice and The Light Surgery Primary Care Network (LSMP &TLS PCN Ltd) are seeking to recruit an enthusiastic and forward-thinkingClinical Pharmacist.This is an exciting opportunity to join a supportive and innovativemultidisciplinary team, working alongside the PCN Senior Clinical Pharmacist,the established pharmacy team and PCN Clinical Directors. The role offerssignificant involvement in direct patient care, with a focus on structuredmedication reviews and medicines optimisation.While experience in primary care and service development is desirable, it isnot essential. The successful candidate will be supported to complete the CPPEPrimary Care Pharmacy Education Pathway if not already undertaken, withdedicated mentoring from the Senior Clinical Pharmacist and GPs. Support willalso be provided for those wishing to pursue Independent Prescribing (IP)qualifications following completion of CPPE Primary Care Pharmacy EducationPathway.
Main duties of the jobThe post holder will:
- Work independently and collaboratively to deliver high-quality pharmaceutical care.
- Carry out complex polypharmacy medication reviews and structured medication reviews particularly for patients with long-term conditions.
- Support the PCN and surgery vaccination programmes
- Provide expert advice to patients and practice staff on medication and prescribing queries.
- Undertake medicines reconciliation and manage acute prescription requests.
- Contribute to the delivery of the PCN Directed Enhanced Service (DES), QOF, and local prescribing incentive schemes (GPOP).
- Promote the principles of medicines optimisation and contribute to audit and quality improvement initiatives.

Job description Job responsibilitiesKey Responsibilities:
- Provide clinical input into the safe and effective use of medicines within the PCN.
- Undertake structured medication reviews (Level 3) to optimise patient outcomes.
- Support the wider clinical team with prescribing queries and guidance.
- Identify unmet pharmaceutical care needs using risk stratification tools.
- Promote rational and evidence-based prescribing in line with NICE guidance.
- Participate in clinical audits, quality improvement initiatives, and national/local incentive schemes.
- Contribute to the training and development of other healthcare professionals.

Person Specification Qualifications Essential- MPharm or equivalent pharmacy degree
- Registration with the General Pharmaceutical Council (GPhC)
- Applicants must also be able to demonstrate relevant primary care experience during their pre-registration training year.
- Completion of the CPPE Primary Care Pharmacy Education Pathway
- Practicing Independent Prescribing pharmacist
- Collaborative working across organisational boundaries
- Strong knowledge of therapeutics and evidence-based practice
- Experience working as a foundation-level pharmacist (evidenced by portfolio)
- Understanding of Primary Care Networks and general practice
- Ability to manage workload and set healthcare targets
- Strong understanding of primary care prescribing
- Awareness of the wider determinants of health
- Excellent interpersonal, communication, and influencing skills
- Competency in using IT systems for clinical and reporting tasks
- Capable of analysing complex clinical data
- Ability to meet deadlines and work under pressure
- Effective at motivating others and promoting safe practice
- Ability to vaccinate adults in line with national immunisation programmes and local protocols
- Confidence in handling cold chain requirements and vaccine-related documentation
- Familiarity with EMIS and/or SystmOne clinical systems
- Confident, polite, and professional
- Collaborative and team-oriented
- High levels of integrity and reliability
- Able to work under pressure and remain resilient
- Strong communication skills, including the ability to explain complex information to patients and colleagues
