PCN Clinical Pharmacist - Medicines Optimisation & Care

The NHS is offering an exciting opportunity for a full-time Clinical Pharmacist to join our Long Lane PCN in Hayes on a permanent contract. The successful candidate will be integral in developing processes for medication management and supporting patient care across multiple practices.

In this role, you will work collaboratively with a multidisciplinary team to ensure optimal medication use, manage chronic conditions, and educate both patients and practice staff.

The starting salary is between £48,000 to £62,000, depending on experience.
