Job Description
PCN Advanced Clinical Practitioner Nurse As an advanced practitioner this post holder will workcollaboratively across the PCN to play a crucial role in clinical governance,building capacity and capability and providing resolution and critical thinkingto some challenging patient diagnosis or issues.
Whilst operating at an advanced practitionerlevel you will drive forward service improvements and clinical governancealongside seeing patients face to face and doing care homes ward rounds andhome visits. The PCN will benefit from your level of knowledge, skills,experience, and leadership to build capacity and capability across the network.
Main duties of the job To assess and triage patients,including same day triage, and as appropriate provide definitive treatment(including prescribing medications following policy, patient group directives,NICE (national) and local clinical guidelines and local care pathways) or makenecessary referrals to other members of the primary care team.
To manage undifferentiatedundiagnosed condition and identify red flags and underlying serious pathologyand take appropriate action.
To use complex decision makingto inform the diagnosis, investigation, complete management of episodes of carewithin a broad scope of practice.
To actively take a personalisedcare approach and population centered care approach to enable shared decisionmaking with the presenting person.
To complete the relevanttraining in order to provide multi-professional clinical practice and CPDsupervision to other roles within primary care, for example first contactpractitioners and the personalised care roles.
To provide specialised clinicalknowledge to assess patients, order and interpret diagnostics and initiatetreatment.
To provide skilled teamleadership to clinicians within the wider Multidisciplinary team.
To provide a high level ofclinical expertise.
The post holder is anautonomous practitioner who will manage a caseload of patients, using evidence-basedpractice/patient focused principles to assess, plan, implement and evaluate interventions.
About us GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN).Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre and the Ryan Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.
As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.
This includes the following areas of focus:
anticipatory care
cardiovascular disease prevention and diagnosis
enhancing health in care homes
early diagnosis of cancer
tackling neighbourhood inequalities
structured medication reviews
social prescribing reviews

To ensure Shared Decision Making (SDM) is embedded within the service and evidenced.
To communicate appropriate complex information effectively to patients and/or carers where there may be barriers to understanding.
To inform them of the benefits and risks of suggested treatment options and to undertake this with groups or individuals.
To demonstrate a sound reasoning of clinical governance, clinical risk and to demonstrate the use of treatment plans with an evidence-based link.
To use advanced clinical reasoning skills enabling formal supervision, specialist support and advice for the AARS roles on a regular basis and be available to advise other staff on the management of complex patients as appropriate, in area of specialist expertise.
Building capacity through effective leadership
To provide clinical leadership to the AARS team, provide 1:1 and carry out appraisal and training plans when needed.
Management of workload and personal development
Act on own initiative in prioritising and managing workload, delegating tasks appropriately and referring to MDT Members in respect of clinical, managerial, and administrative areas, as necessary.
Enrolled into review and appraisal systems within the PCN.
Maintain your own personal and professional development as agreed at your annual review.
Manage own time, workload, and resources effectively.
Collaborative Working Relationships
Recognises the roles of other colleagues within the organisation and their role to patient care.
Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations)
