Their interdisciplinary teams work closely with all stakeholders in the system, including family doctors, hospitals, and retirement homes. Main responsibilities: Ensure follow-up with partners and become their primary contact Respond to their requests with responsiveness, professionalism, and actively work on their sustainability Process new registrations sent by partners Create files in the system ensuring compliance and the validity of medical prescriptions according to the requirements of different health insurance companies Contact individuals to organize the first delivery and explain the order process to them Contact individuals each month to verify adherence to treatment, organize the monthly delivery of pharmaceuticals and/or nutrition, and transmit, with their consent, the necessary details to the responsible dietitian Ensure nutritional follow-up of stable patients at the request of partners by assessing weight changes and treatment effectiveness, adjusting treatment if necessary, in coordination with the dietitian or treating physician Establish and send invoices to health insurance companies and/or respond to refusals or returns from health insurance companies. jidc5a9108aen jit0310aen jpiy26aen
