Job Description
You will be working across the Breightmet and Little Lever PCN practices with other paramedics already employed by the GP Federation
All Working hours will be between 08:00AM and 6:30PM Monday to Friday
Main duties of the job Main duties of the job
You will assess and treat patients for both acute and chronic health problems;You will see patients in person either in the GP practice, care and residential homes, or the patients own home, you may be asked to undertake telephone consultations;To screen patients for disease risk factors and early signs of illness;To actively pursue policies that will promote healthy lifestyles for patients: promote health education and participate in relevant sessions within the practice.Support practices in working towards local and national health care targets such as QOF.You will discuss patient-related concerns with the relevant staff, and where required, you will be supported to follow appropriate safeguarding procedures;When needed you will order and collect pathological specimens including blood samples, and in monitoring physiological observations using a structured approach along with ordering investigations;You will administer medication and vaccinations within the relevant Patient Group Directions, or, if qualified and appropriate, via Independent Prescribing;You will continue to develop and maintain your advanced clinical skills and continuously improve personal specialised skills through audit, research, review and fulfilling training needs. You will have a key role in the delivery of the Network Contract DES specifications, and you will support the coordination and delivery of Multi-Disciplinary Teams where needed.

Job responsibilities PARAMEDIC To provide paramedic practitioner activities in a primary caresetting to support the GPs, holding clinics and visiting patients within theirown homes or visiting local care homes;
To treat both patients with acute and urgent problems as well asreview chronic diseases and ongoing health issues as part of both local andnational primary care performance targets such as Quality Outcome Framework(QoF);
To actively pursue policies that will promote healthy lifestylesfor patients: promote health education and participate in relevant sessionswithin the practice;
Ensure the clinical records are up to date with accurate detailsrecorded and amended, including appropriate use of SNOMED codes;
To screen patients for disease risk factors and early signs ofillness;
You will be competent in triage, in collecting pathologicalspecimens including blood samples, and in monitoring physiological observationsusing a structured approach along with ordering investigations;
You must be able to assess and diagnose patients both remotely andin person;
You will discuss patient-related concerns with the relevant staff,make onward referral if required; and when needed, you will be supported tofollow appropriate safeguarding procedures;
You will administer medication and vaccinations within the relevant Patient Group Directions, or, if qualified and appropriate, viaIndependent Prescribing;
You will continue to develop and maintain your advanced clinicalskills and continuously improve personal specialised skills through audit,research, review and fulfilling training needs;
You will work as part of a diverse team, collaborating withcolleagues across the Network to provide the best care to our patients;
To develop with the patient an on-going nursing care plan forhealth;
You will ensure safe handover of care within and outside the GPPractices as appropriate;

Advanced Clinical Practitioner The Advanced Clinical Practitioner works autonomously and isaccountable for own professional actions.
Undertakes Clinical Practice at an advanced level using expertknowledge and clinical skills to deliver holistic care to people accessingprimary health care services;
Assesses diagnoses and treats patients attending primary healthcare services with a range of acute, non-acute and chronic medical conditionsas part of both local and national primary care performance targets such asQuality Outcome Framework (QoF);
Assesses diagnoses and treats patients in surgery, care homes orpatients own home who require acute medical attention and refer onwards whenappropriate;
Directly admits patients to secondary care hospital in acutemedical need;
Refers patients to other medical specialities for assessment ifdeemed appropriate;
Refers patients as appropriate to other members of themultidisciplinary team, secondary care and to other statutory and voluntaryorganisations;
Ensure the clinical records are up to date with accurate detailsrecorded and amended, including appropriate use of SNOMED codes;
Undertakes and interprets a range of diagnostic tests and routineclinical procedures;
Makes critical judgements of the highest order to satisfy theexpectations and demands of the job e.g. first contact with patients presentingacutely and manages care in the presence and absence of protocols;
Makes decisions where precedents do not exist, where appropriatewithout recourse to others;
Advises and supports others where standard protocols do not apply;
Works within professional guidelines and codes of conduct;

- Professional registration (NMC, HCPC etc.) MSc in Advanced Clinical Practice. Evidence of CPD Evidence of teaching or assessing in clinical practice. Mentorship or teaching qualification. Degree or Level 7 equivalent
- Advanced clinical practice skills Management of patients with common acute medical conditions Management of patients with long-term conditions Knowledge of the NHS Understanding of the current issues and challenges facing primary care
- Broad range of clinical experience in a variety of settings with significant experience at Band 6 or above. Experience of working with a degree of autonomy and high level clinical decision making and accountability. Experience in taking patient history. Able to undertake assessment and critically analyse to make safe clinical decisions. Experience of multi-disciplinary working Demonstrable competency in wide range of clinical skills and willingness to learn. IT literacy
