What You Will Do:

• Conduct thorough assessments to identify functional limitations and determine individual care needs.
• Deliver services in alignment with Care Act 2014 legislation and ensure comprehensive care act reviews are completed.
• Collaborate effectively within a multidisciplinary team to ensure the best outcomes for clients.
• Apply safeguarding principles and the Mental Capacity Act to safeguard vulnerable adults.
• Develop creative interventions tailored to the unique needs of each individual.
• Communicate clearly and effectively with clients, families, and colleagues to facilitate excellent care.
• Manage your caseload efficiently with strong organisational and time management skills.
