We are currently seeking a dedicated and experienced Occupational Therapist to join a dynamic multi-disciplinary team of Social Workers, Physiotherapists, and Occupational Therapists delivering an integrated health and social care service to the local community in Islington.
Location:Islington, North London
Hourly Rate: £32.02
Contract Type: Locum/Agency - Hybrid Working
Working Pattern: Full-time
About the Role: As an Occupational Therapist, you will take a lead role in coordinating care and support for service users, carers, and families-working closely with other professionals to ensure the best outcomes. You will manage a caseload of varying complexity and help people maintain or regain their independence through holistic and person-centred interventions.

Key Responsibilities:
- Manage and care coordinate a caseload of adults with diverse and complex needs
- Work collaboratively within the multi-disciplinary team to provide integrated care
- Liaise with service users, carers, families, and professionals to develop and implement care plans
- Support and guide junior staff on more complex cases (if at higher grade)
- Accurately input and maintain up-to-date case management records for monitoring and planning
- Provide a cost-effective, person-centred service using available resources across Social Services, Health, community, and voluntary sectors
- HCPC registered Occupational Therapist
- Previous experience working within a community or social care setting
- Strong communication, organisational, and case management skills
- Ability to work autonomously and within a team environment
- Experience managing complex cases and contributing to multidisciplinary planning
