Job Description
Towork autonomously, the NP/ANP is responsible for consulting with patients who wishto be seen by a health care professional; diagnosing and treating patientspresenting with minor illness; seeing and advising people in respect of theircontinuing medical and nursing needs. All consultations will be recorded on SystmOne,the clinical IT system. NP/ANP is required to conduct both telephone and F2F appointments. Intermittently there is a requirement to see extra oremergency patients and carry out home visits for an agreed cohort of patients. The nurse practitioner will need toprioritise and triage the needs of patients accordingly, making any necessaryreferrals for investigations in a timely appropriate manner. Patients withconditions requiring referral to secondarycare are discussed with the registered GP before making such a referral.

Main duties of the job Diagnosingand treating patients presenting with minor illness

Triage andtreat patients presenting with minor illnesses to see a health careprofessional, making any necessary referrals to other members of the primaryhealth care team.

Ensureclinical practice is safe and effective and remains within boundaries ofcompetence and to acknowledge individual limitations.

Advisepatients on general health care and minor ailments with referral to othermembers of the primary and secondary health care team as necessary.

Undertakethe collection of pathological specimens including intravenous blood samples,swabs etc. Perform appropriate investigatory procedures requested by the GPs.

Administration andprofession responsibilities

Participatein the administrative and professional responsibilities of the practice team.

All patientactivities are completed daily.

Ensureaccurate and legible notes of all consultations and management plans arerecorded in the patients notes on the clinical system, SystmOne.

Ensureaccurate completion of all necessary documentation associated with patienthealth care and registration with the practice.

Ensurecollection and maintenance of statistical information required for regular andad hoc reports and audit.

Attend andparticipate in practice meetings and mandatory training sessions.

Undertakethe on line training and other training pertinent to the role of NP or ANP as agreedwith the Practice Manager or Partners.

Thepost-holder will participate in any training programme implemented by thepractice as part of this employment, such training to include:

Participationin an annual individual performance review, including taking responsibility formaintaining a record of own personal and/or professional development.

Takingresponsibility for own development, learning and performance and demonstratingskills and activities to others who are undertaking similar work.

Maintaincontinued education by attendance at courses and study days as deemed useful ornecessary for professional development, ensure PREP requirements are met.

If it isnecessary to expand the role to include additional responsibilities, fulltraining will be given.

As well asthe nursing team, there is a need to work closely with reception and adminstaff to ensure the smooth running of the practice, reporting any problemsencountered to the relevant person and ensuring everyone is aware of thedifferent roles within the nursing team.

There isalso the need to establish and maintain good liaison with other surgeries andagencies, including secondary care.

Meetings

It will be necessary to attend and contributeto relevant practice meetings as requested.

Health & Safety:

Thepost-holder will participate in a full range of promotion and management oftheir own and others health and safety and infection control, as defined inthe practice Health & Safety policy, the practice Health & Safetymanual, and the practice Infection Control policy and publishedprocedures. This will include (but willnot be limited to):

Usingpersonal security systems within the workplace according to practiceguidelines.

Awareness ofnational standards of infection control and cleanliness and regulatory /contractual / professional requirements, and good practice guidelines.

Responsiblefor correct and safe management of the specimens process, including collection,labelling, handling, use of correct and clean containers, storage and transportarrangements.

Managementand maintenance of Personal Protective Equipment (PPE) for the practice includingprovision, ordering, availability and ongoing correct usage by staff.

Ownership ofinfection control and clinically based patient care protocols andimplementation of those protocols across the practice.

Activeobservation of current working practices across the practice in relation toinfection control, cleanliness and related activities ensuring that proceduresare followed and weakness / training needs are identified, escalating issues asappropriate.

Identifyingthe risks involved in work activities and undertaking such activities in a waythat manages those risks across clinical and patient process.

Makingeffective use of training to update knowledge and skills, and initiate andmanage the training of others across the full range of infection control andpatient processes.

Monitoringpractice facilities and equipment in relation to infection control along withother members of the nursing team, ensuring that provision of hand-cleansingfacilities, wipes etc. are sufficient to ensure a good clinical workingenvironment. Lack of facilities to beescalated as appropriate.

Safemanagement of sharps procedures, including training, use, storage and disposal.

Usingappropriate infection control procedures, maintaining work areas in a tidy,clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective actionwhere needed or escalation to responsible management.

Actively identifying, reporting, and correctionof health and safety hazards and infection hazards immediately when recognized.
