Nurse assessor all age continuing care

The post holder will be responsible for assessing and determining eligibility for people with complex health needs who are eligible for NHS funding for long term care and treatments under The National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, The Mental Health Act. The service also commissions packages of care for clients requiring Transforming Care and Neurological Rehabilitation pathways.
The post holder will be responsible to clinically plan and commission the care required, ensuring that the care is safe, effective and good quality. Communication and relationships with individuals and independent providers is a key part of this role as well as abilities to manage risks, problem solve influence and negotiate to ensure appropriate clinical commissioning case management of complex individuals.
The post holder will chair Multi-disciplinary meetings in order to make eligibility recommendations for NHS Continuing Healthcare and NHS Funded Nursing Care. Complete reviews of commissioned packages ensuring that people are receiving high quality safe and effective care as well as ensuring they still have eligibility for the relevant funding stream. You will also provide expert advice to other professionals commissioned to provide care in relation to NHS continuing Healthcare and NHS Funded Nursing Care packages.
Main duties of the job- The post holder will act as Individualised Nurse Assessor for patients whose care is either jointly or solely funded by NHS GM and the care is provided by the independent sector.
- Identify and case manage clients who may be eligible for NHS funding, chair and coordinate eligibility assessments and review.
- Provide expert advice to other professionals commissioned to provide care in relation to NHS Individualised Commissioning.
- Complete the restitution process for all claims which includes presentation of findings to the NHS GM multi-agency funding panel and the NHS England Independent review panel.
- Contribute to the quality monitoring of care home providers
- Co-ordinate and manage multi-disciplinary meetings ensuring all appropriate information and evidence is obtained & considered to make decisions regarding eligibility for NHS Continuing Healthcare
- Complete concise and accurate reports
- Understand and communicate complex information, both patient and non- patient related
- Communicate complex and often sensitive and distressing information on a regular basis.
- Manage complex, sensitive and sometimes contentious situations
- Contribute to the safeguarding and quality agendas
- Establish partnerships and networks with partner agencies
- Maintain an educational and advisory role to others in relation to NHS Continuing Healthcare, Funded Nursing Care and Personal Health budgets.
- Maintain up-to-date, accurate records, inputting case management into the electronic patient system
- Undertake duties in a timely manner whilst managing frequently competing demands on a daily basis.
- Have the ability to work under pressure within an agreed time frame
- Engage in clinical supervision and self-reflection to develop own professional standards and working in practices
- Undertake assessments in accordance with the National Framework using clinical knowledge and expertise to assess patients health care needs and determine care requirements
- Have the ability to positively challenge and influence health and social care professionals to ensure that plans for patient care are appropriate and that the patient will be cared for on the most appropriate pathway where possible utilising the least restrictive options. Act appropriately when there are requirements to re consider care provision. Make recommendations of changes to provision and interim solutions if required.
- Ensure fair and consistent application of the NHS National Framework
- Accountable for the appropriate use NHS Resources
- Registered Nurse with current NMC registration
- BSC (Hons) Degree in Nursing or related subjects or relevant experience
- Significant nursing experience at band 5 or equivalent experience
- Evidence of post qualifying and continuing professional development
- Experience of working as a Registered Nurse with patients with multiple/complex needs
- Responsibility for a defined caseload which includes assessment, planning, implementation and evaluation of nursing care
- Experience in working within a multi-disciplinary team setting
- Experience of clinical supervision and reflective practice discussions
- Experience of working with Older People with Mental Health difficulties
- Knowledge and understanding of current national policy/framework guidance relating to NHS Continuing Healthcare and NHS Funded care.
- Awareness of current legislation relating to health and social care
- Understanding of the current issues impacting on the NHS
- Confidence to manage complex multi-disciplinary.
- Knowledge of retrospective reviews
- Ability to build effective partnerships with a wide number of partners and stakeholders
- Effective negotiating and influencing skills
- Use effective verbal communication skills with a wide range of individuals including patients and families/representatives, health and social care professionals, commissioners and private and public sector care providers
- The ability to lead multi professional MDT's
- Ability to assess and re prioritise own work
- Ability to change plans and respond effectively to meet service demands
- Adaptability, flexibility and ability to cope with uncertainty and change or challenging situations
- Ability to engage with and learn from peers, other professionals and colleagues in the desire to provide or support the most appropriate interventions.
- Experience of involvement in Safeguarding Investigations
- Must be willing to work flexibly in response to client and organisational needs.
