Mental Health Social Worker - Discharge Team (Hybrid)

West Sussex County Council is recruiting a Mental Health Social Work Discharge Team Social Worker/Senior Social Worker to conduct strengths-based assessments and develop support plans that improve wellbeing and outcomes.
