Job overview

CATAT was highly recommended at the staff excellence awards after its launch in 2023. We seek professionals interested in working with CYP and their families to improve outcomes and maintain community care.

The team aims to prevent inpatient admissions where possible and supports CYP and families proactively in the community. It acts as a gateway for Tier 4 (T4) services and collaborates with core community teams and partner agencies to enhance support, risk management, and continuity of care.

CATAT has two main pathways:

  1. Admission Avoidance: For CYP already known to a core team needing intensive support to prevent worsening or hospital admission.
  2. In-reach/Outreach: Supporting CYP transitioning from inpatient settings back into the community.

Between 07/2022 and 08/2024, our team has avoided 165 admissions. If you're dynamic and passionate about supporting young people and families, apply now to join us!
