Lewisham and Greenwich NHS Trust, (LGT), is a community-focused provider of local and acute care, delivering high-quality services to over one million people living across the London boroughs of Lewisham, Greenwich, and Bexley. Are you a passionate and experienced nurse with significant expertise in the assessment and management of patients with leg ulcers, lymphoedema and chronic oedema?

We are looking for an enthusiastic and dynamic clinician to lead our specialist Lymphoedema and Leg Ulcer Service. This is an exciting opportunity for an innovative nurse leader who is committed to delivering high-quality patient care, developing services, and supporting the NHS vision of delivering more care closer to home.

As Team Leader, you will provide clinical and operational leadership to a small, dedicated team of specialist nurses, ensuring the delivery of evidence based, patient centred care across community settings. You will play a key role in improving outcomes for patients living with complex lower limb and lymphatic conditions while supporting self management and prevention.

Working closely with colleagues across primary care, community services, acute providers and wider stakeholders, you will help drive service improvement and strengthen integrated working across the system.

Main duties of the job

• Lead and support a specialist team delivering high-quality lymphoedema and leg ulcer services.
• Provide expert clinical assessment, treatment planning and advice for patients with complex lower limb and lymphatic conditions.
• Develop strong relationships with internal and external partners to promote integrated care pathways.
• Act as a clinical expert and resource for healthcare professionals across the health and care system.
• Deliver education, training and mentorship to staff, partners and stakeholders.Contribute to service development, quality improvement and innovation.
• Support the development of community based models of care aligned with national NHS priorities.

Detailed job description and main responsibilities

To provide a high-quality service across Lewisham and Greenwich Healthcare NHS Trust
through line management of the Leg Ulcer/Lymphoedema Specialist Nurses and to
demonstrate the required skills in Lymphoedema and leg ulcer management along with expert
assessment skills while working in close collaboration with other clinical colleagues to lead the
development in the delivery of an expert nurse-led service across the organisation. The post
holder will contribute to advancing the body of knowledge through education, research and
audit with the aim of improving access to treatment services. The post holder will have a key
role in ensuring seamless, co ordinated and timely approach to care for their patient group in
clinics and patients own home as appropriate. The post holder will also provide leadership,
management and a high level of clinical expertise in their specialism and contribute to the
support, education and training for their team and other healthcare professionals. The post
holder will have a clear understanding of the strategic political view and challenges faced by primary, community and secondary care through the STP in meeting recommendations for the development of the service and will work collaboratively with the multi disciplinary team in the
planning and development of a more integrated service for patients across primary, community and secondary care.
This post will include working and travelling to all Trust sites.
