Locum Consultant in Older People's Medicine

The Leeds Centre for Older People's Medicine is seeking to appoint a Locum Consultant Geriatrician. The successful candidate will be working as part of the Older People's Medicine team delivering inpatient care for patients based within our orthogeriatric wards at the LGI, delivering Bone Health clinics and outlier wards within the Trust. There may be the opportunity to provide some sessions on our same day emergency care unit, and this can be discussed at appointment. The post will be 10 PAs but we are willing to discuss less than full time for any potential candidates that are interested. The initial appointment will be for 6 months with a view to a longer period of time if successful.

The successful candidate will join an enthusiastic and supportive Older People's Medicine team which has 31 consultant colleagues, supported by a team of qualified and trainee ACPs and 5 substantive PAs.

Main duties of the job

The post focuses on the delivery of care of Older People.

To ensure high quality and safe care of elderly medical patients resulting in excellent clinical outcomes.

To contribute to the care of patients on the older people's orthogeriatric and specialty wards thereby extending Geriatrician presence throughout the usual working week.

To be able to contribute to the out-of-hours provision of elderly medicine on a rota.

To show clinical leadership in patient care, service development and clinical governance.

To work co-operatively and effectively as part of the wider elderly medicine team, being held to account for clinical, service development and training outcomes.

Applicants must be able to demonstrate a high level of clinical experience and competence in all aspects of Older People's medicine. Excellent communication and team working skills, a strong commitment to teaching and service development are essential.

The successful applicants should hold accreditation in Membership of the Royal College of Physicians or equivalent, and should be on the Specialist Register.

The post will be based at St James's University Hospital, and will be predominantly clinical with responsibility as a member of consultant team for providing inpatient care for people living with medical and frailty conditions. Consultant mentoring will be offered.

Job responsibilities

The post will be based at both SJUH & LGI. The Elderly Medicine admission wards are based in Gledhow Wing, St James Hospital, and have between 26-29 beds each. At least two consultants conduct a post-take ward round on each admission ward during the week. Each ward is supported by a registrar, two core medical trainees or FY2 trainees and one FY1 trainee.

On-call duties: Monday to Thursday the Elderly Medicine consultants are on call on a 1:20 rota. During the winter months, three Elderly Medicine consultants take part in ward rounds on the acute admissions wards. Only one of these consultants is on call overnight. In the summer months, two Elderly Medicine consultants conduct the weekend ward rounds. From 2018 an evening ward round between 17:00 and 20:00 would be expected of the first on-call consultant. The postholder would work six weekends in a 12-month period.

The post holder will work with the Orthogeriatric and orthopaedic teams to provide evidence-based high quality care for older people presenting with a fracture, with particular focus on fractured neck of femur, perioperative medicine and bone health. They will link with consultant colleagues in other specialist teams within the Trust in particular the emergency department, other medical and acute surgical specialties and with colleagues in primary care and those at the interface to provide excellent care for older people living with frailty.

The postholder would spend time in the SDEC Unit in the Emergency Department alongside the Leeds Integrated Discharge Service (LIDS) to help identify and assess patients who can be discharged appropriately direct from the ED.

Each of the Elderly Wards in Gledhow Wing, St James Hospital, have between 27 and 30 beds and the postholder would share their Gledhow Wing with other consultants. The consultants on each ward would be expected to cover each other's patients when their colleague is on leave. Patients under the care of the Elderly Medicine department in beds outside of the CSU are divided up between the consultants depending upon which outlying ward the patients are on.

CommunityCare Bed (CCB) unit: There are several rehabilitation units around Leeds which take admissions from the hospital and community. There is a GP VTStrainee based there to support the post holder.

Neighbourhood work: Leeds is divided up into 13 neighbourhoods bringing both health and social care services together to improve services in the community. The neighbourhood teams are made up of district nurses, community matrons, intermediate care professionals and social workers along with the existing GP practice staff. The Elderly Medicine consultant supports these teams by giving advice at the team meetings and doing domiciliary visits if required.

Person Specification Eligibility, Fitness to practice, and Language Skills

  • All applicants to have demonstrable skills in written and spoken English adequate to enable effective communication about medical topics with patients and colleagues

  • Service Development Experience. Attendance on a recognised management course
Qualifications
  • Higher degree e.g. MD/PhD relevant to the subspecialty

  • Must be able to demonstrate a high level of clinical experience and competence in older people's medicine. Evidence of continuous career progression consistent with personal circumstances.
  • Experience of participation in regular clinical audit
