Job overviewAn exciting opportunity has arisen for a motivated and enthusiastic Mental Health Practitioner to continue their professional development within the Living Well Team in Bury.
You will be supporting and, playing a key part in, the establishment of the new service model. The Living Well Team offers a new way of working and engaging with residents of the borough.
The Practitioner will be embedded in one of our neighborhoods and be a part of a small team delivering interventions in the area.
Main duties of the jobThe role will involve participating in regular MDT Discussions to screen, formulate and provide care for the residents of your neighborhood.
Working closely with partner services and third-sector organisations the role will involve screening referrals, offering holistic assessments, brief interventions and formulation at a primary care level.
It will also incorporate offering assessments to local residents presenting with acute distress and developing risk formulation and management plans.
It will involve functioning as a Duty Worker, offering specialist advice and support to external agencies, such as GP's, Social Work Teams and third-sector colleagues.

Person specification Education/Qualifications- Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification or willingness to complete
- Evidence of on-going CPD
- Further training in brief interventions.
- Proven experience of working with clients with complex needs
- Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness
- Experience of working in multidisciplinary teams.
- Primary care mental health work experience
- Experience of screening referrals
- Understanding of current mental health legislation, Safe Guarding and Child Protection Legislation
- Awareness of NICE Guidance in the treatment of mental health problems and knowledge of treatments and interventions appropriate for individuals with mental health needs
- Understanding of confidentiality and data protection issues
- Knowledge of psycho-social assessment and risk assessment / management processes
- Comprehensive Risk Assessment and Formulation Skills
- Ability to construct effective care and risk management plans
- Ability to deliver a range of evidence based interventions
- Ability to form productive working relationships with service users, carers and multi-disciplinary staff.
You must have appropriate UK professional registration.
