An exciting secondment position has arisen to join the Inflammatory Bowel Disease (IBD) Nursing Teamas a Band 6 Clinical Nurse Specialist. They will be an integral member of the IBDteam, which provides care and treatment for patients with inflammatory boweldisease in an inpatient and out patient setting. This is a development opportunityto develop your expertise in specialist IBD care, working closely with IBD teamand wider multidisciplinary team. Youll play an important role in supportingpatients through diagnosis, treatment, flare management and ongoing diseasemonitoring, helping them to understand and take an active role in managingtheir condition. Youll have the opportunity to develop advanced clinicalknowledge, including assessment, treatment pathways, medicines management,biologic and advanced therapies, patient education and telephone or virtualclinical support. Youll use your clinical judgement to recognise deteriorationand disease activity, coordinate appropriate care and provide timely advice andsupport.

It is essential for the post holder to develop strongprofessional relationships with colleagues, Trust employees, and staff fromexternal agencies.

The IBD nurse specialist will be flexible to meet the needsof the team and will demonstrate knowledge, expertise and passion in workingwithin the speciality of Inflammatory bowel disease.

Main duties of the job

To assess and support patients with IBD, includingrecognising symptoms of active disease, complications and deterioration. To providespecialist advice and education to help patients understand and managetheir condition.

To support patients receiving biological andadvanced therapies, including treatment monitoring, safety checks and ongoingreview.

Responding to patient concerns and providing inpatientsupport, out patient clinics, telephone, virtual and face-to-face advice,helping patients access the right care at the right time.

Work collaboratively with gastroenterologists,surgeons, dietitians, pharmacists and other members of the multidisciplinaryteam to coordinate holistic care.

Supporting patients through flare-ups,investigations, treatment changes and periods of remission. Monitoringtreatment response and contributing to clinical assessment, care planning anddisease monitoring. Recognising when patients need escalation and ensuringconcerns are communicated promptly to the appropriate clinician or service.

Contributing to clinical audits, research, qualityimprovement and service development, helping us continually improve IBD care.

To provide a continuouslyimproving service in line with service requirements.

To communicate effectively with all members of themulti-disciplinary team, patients, carers and the public.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Person Specification Qualifications

• Diploma in Nursing
• Degree in Nursing or working towards
• Teaching qualification
• Clinical skills course

• High level of interpersonal and communication skills

• RN NMC Registration

Experience

• Broad knowledge, skills and experience in gastroenterology nursing of 18 months.
• Evidence of ongoing Professional Development and Clinical Practice Development (CPD)

• Ability to cope with and diffuse difficult and stressful situations
• Experience of multi-professional working
• Relevant experience in specialist area
• Experience of change management
