Job Description
GP Partner (5sessions) Central Cornwall High-performing, supportive practice
Chacewater &Devoran Surgeries are looking for a GP Partner (5 sessions) to join ourfriendly, stable team from September.
We are a well-established, dispensing practice (2 sites) with 4 partners and 1 salaried GP, serving 7,000 patients in a beautiful semi-rural setting near Truro and Falmouth.
Main duties of the job To provide medical services to the practice population, delivering an excellent standard of clinical care.
The successful candidate will help provide effective management of the practice, leading by example and maintaining a positive and collaborative working relationship with fellow partners and our practice team.

Job responsibilities There will be a requirement to carry out othertasks; this will be dependent upon factors such as workload and staffinglevels:
The delivery ofhighly effective medical care to the entitled population
The provision ofservices commensurate with the GP contract
Clinically examineand assess patient needs from a physiological and psychological perspectiveand plan clinical care accordingly referring patients to otherservices/agencies in a timely manner, utilising practice and any other localguidelines
Prioritise healthproblems and intervene appropriately to assist the patient in complex, urgentor emergency situations, including initiation of effective emergency care
Effective managementof long term conditions
Support patients toadopt health promotion strategies that promote healthy lifestyles and applyprinciples of self care
Processadministration in a timely manner including referrals, repeat prescriptionrequests and other associated administrative tasks
Review medicationsfollowing the appropriate policies, NICE/local clinical guidelines and localcare pathways
Maintain accurateclinical records in conjunction with good practice, policy and guidance
Work collaboratively,accepting an equal share of the practice workload
Adhere to bestpractice recommended through clinical guidelines and the audit process
Support the delivery of anticipatory care plans
Take responsibilityfor continuous improvement and quality initiatives within the organisation
Accept delegatedresponsibility for a specific area (or areas) or the QOF
Attend and contributeeffectively to external and internal meetings as required
Contributeeffectively to the development and maintenance of the organisation includingclinical governance, training, financial management and HR
Be aware of dutiesand responsibilities regarding current legislation and adhere to practice policiesand procedures on safeguarding adults and safeguarding children
Ensure compliancewith the appraisal process
Prepare and completethe revalidation process to remain fit to practice
Commit toself-learning and instill an ethos of continuing professional developmentacross the organisations team
Review and adhere topractice protocols and policies at all times
Encourage collaborative working, liaising with all staff regularly,promoting a culture of continuous improvement
Be an integral partof the general practice team as well as supporting the wider network andmulti-disciplinary team
On a rotationalbasis, undertake telephone triage and duty doctor roles
Accept responsibilityfor a specialist interest within the organisation
Contribute to the effective management of the organisation includingadministration, planning, strategy, resilience planning etc.
Identify, implement and embed best practice through clinicalguidelines and the audit process
Contribute to the training of medical students from all clinicaldisciplines
Show leadership qualities, work collaboratively, ensure an evendistribution of the organisations workload
Remain fully focused on the strategic aims of the organisation, makingrecommendations to enhance income and reduce expenditure
Prepare and attendpartnership meetings contributing in a positive manner
Provide proactive leadership to the organisation, patients and theircarers
Support in the delivery of enhanced services and other servicerequirements on behalf of the organisation
Participate in the management of patient complaints when requested to do so, and participate inthe identification of any necessary learning brought about through clinicalincidents and near miss events
Undertake allmandatory training and induction programmes
Contribute to and embrace the spectrum of clinical governance
Attend a formal appraisement at least every 12 months. Once a performance/training objective hasbeen set, progress will be reviewed on a regular basis so that new objectivescan be agreed
Contribute to public health campaigns (e.g., flu clinics) throughadvice or direct care
In addition to the primary responsibilities,the Partner has the following wider responsibilities:
Participatein formal training events promoting best practice in area of expertise Provideleadership and education for members of the multi-disciplinary team, providingguidance and support when necessary
Participatein local initiatives to enhance service delivery and patient care

- Outstanding level of clinical knowledge and skills commensurate with that of a Partner
- Understanding the importance of evidence-based practice
- Good MS Office and clinical system IT knowledge

- An appreciation of the NHS landscape and relationship between individual organisations, PCNs and the commissioners

- Occupational Health clearance
- Flexibility to work outside of core office hours
- Evidence of continuing professional development (CPD) commensurate with the role
- Access to own transport and ability to travel across the locality on a regular basis
