Job Description

To deliver high-quality primary care services as part of the Primary Care Network (PCN), supporting improved patient access, health outcomes, and system efficiency in line with the Network Contract DES, with a focus on capacity and access.

Main duties of the job

Clinical Duties

Provide a full range of GP services, including

  • Face to face consultations
  • Telephone and video consultations
  • Home visits where appropriate
  • Diagnose and manage acute and chronic conditions
  • Prescribe in accordance with local and national guidelines
  • Maintain accurate and contemporaneous clinical records
  • Support the practice using online consultation tools and Total Triage
  • Will include on call duties

PCN & ARRS Role-Specific Duties

  • Support enhanced access and same-day care provision
  • Contribute to:
    • Holistic care
    • QOF achievement
    • Structured medication reviews (where appropriate)
    • Care of patients with complex needs
    • Support delivery of PCN service specifications

Job responsibilities

We are seeking an enthusiastic and committed General Practitioner (GP) to join our Primary Care Network (PCN) under the Additional Roles Reimbursement Scheme (ARRS).

This is an exciting opportunity to work collaboratively across our network, supporting the delivery of high quality, patient centred care while helping to meet the evolving needs of our population.

  • Work within the practice to improve access and continuity of care
  • Deliver a mix of face to face, telephone, and digital consultations
  • Support proactive and preventative care initiatives
  • Contribute to multidisciplinary team (MDT) working
Qualifications
  • Fully qualified GP
  • Registered with the GMC and on the GP Performers List
Experience
  • Experience in UK General Practice
