The Medical-Social Centers have the mission to help individuals of all ages who are dependent or have health issues to remain in their living environment for as long as possible and to support family caregivers. ASPMAD, with its 9 CMS, cares for clients at home annually and serves 105 municipalities in the North. We are looking for a dietitian for a fixed-term contract with a work rate of 20 to 90%. Your missions: As a Dietitian, your role as a resource person for your future team involves an advanced mastery of specific skills and tasks: Design and implement tailored nutritional counseling services, coordinating with the designated reference person to meet clients' specific needs. Supervise and support teams: Ensure the support of the CMS teams in a dynamic common care approach, including help and home care, as well as the function of sharing: Guarantee a multidisciplinary approach by documenting relevant information in clients' files and facilitating communication among the various stakeholders concerning meal quality: Supervise meals for CMS clients, ensure the quality of meals delivered at home, and conduct thorough follow-up. Management of costs: Assess the costs related to specific diets for the CCAVS, taking into account supplements for the concerned clients. Profile: You have a diploma from a Swiss school of dietetics or equivalent, complemented by at least two years of experience in this role and possess a registration number. You have a clear interest in elderly care and home maintenance, with the ability to work in a multidisciplinary manner. Flexibility and rigor are some of your qualities; you enjoy change, hold a driving license, and have personal transportation.
