Consultant Practitioner for Urgent and Emergency Care

Full time

12 month fixed term/secondment

We are seeking an exceptional Consultant Nurse to join our Integrated Care & Community Services Group on a 12-FTC . This pivotal leadership role will provide strategic and operational oversight for all Urgent and Emergency Care (UEC) pre hospital delivery across Coventry, ensuring high quality, safe, and responsive care for our population.

This is a senior clinical leadership position responsible for driving excellence across our pre hospital UEC services while supporting and developing our multidisciplinary workforce, including:
  • Advanced Clinical Practitioners (ACPs)
  • Trainee ACPs
  • Clinical Practitioners
  • Registered Nurses
The post holder will be instrumental in maintaining clinical standards, enhancing workforce capability, and ensuring robust governance within a dynamic community setting.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job The post holder will be a clinical expert in the management of complex, acute and undifferentiated conditions within primary and community care and will fulfil the 4 domains of consultant practice: expert practice (the consultant's main health or social care profession); strategic and enabling leadership; learning, developing and improving across the system; research and innovation.

The post holder will practice at an advanced clinical level and exercise high levels of judgement, discretion and decision making in clinical care. They will provide clinical leadership, expert practice, and advanced knowledge, integrating research evidence into practice and contributing to/leading on research within the clinical field of community services and primary care.

The post holder will monitor and improve standards of care through supervision of practice, clinical audit, identifying research questions, disseminating research findings and promoting their translation into practice, teaching and supporting professional colleagues and the provision of skilled professional leadership. As a member of the MDT, the post holder will be involved with, and where appropriate collaborate in national and international research contributing to relevant networks and forums, with the aim of supporting the development of an acute care delivered closer home.

Job responsibilities The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.

If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model. However, any formal changes will be preceded by appropriate consultation in line with organisational policy.

Competence in Non Medical Prescribing will be required, if supported by the practitioner's professional regulator.
  • The post holder will have a highly visible clinical profile, providing both advisory and clinical input into patient care, spending 20% of time in clinical practice.
  • Autonomously act and develop recognition for their expertise in the field of primary and community care services having developed specialist clinical knowledge.
  • Apply advanced theoretical, evidence based and practical knowledge to the clinical care of patients with community patients when presenting with an acute/undifferentiated health need.
  • Work collaboratively with multidisciplinary teams to optimise clearly defined patient outcomes.
  • Undertake comprehensive patient assessment within a multidisciplinary team setting to optimise patient outcomes.
  • Provide leadership, guidance and exercise judgement in making challenging patient centred decisions within current best practice guidance, relevant legal frameworks and with clear understanding of the relevant professional and applied ethical frameworks.
  • Benchmark and align patient outcome indicators with national top performers.
  • Ensure that adult safeguarding procedures are adhered to within own practice and service.
  • Meet and communicate the needs of culturally diverse patients.
  • Develop and maintain expertise in highly developed skills and appropriate competencies, ensuring the highest level of precision to maintain patient safety in line with Trust and NMC regulatory frameworks.
  • Identify local, demographic, cultural and environmental factors that influence the health and quality of life for this patient cohort.
  • Identify and implement new developments, treatments and technologies into clinical practice.
  • Promote the translation of evidence based/research findings and best practice guidance into practice.
  • To undertake the role of Independent Nurse Prescriber within the clinical setting and in line with Trust policy / National guidance.
  • Undertake annual audit of prescribing practice.
  • Prepare accurate and concise reports both verbally and written.
  • Deploy effective communication strategies to ensure that information and knowledge is imparted sensitively according to the needs of patients, staff and students in training.
  • Be a member of relevant Trust and Network committees and groups providing expert advice and contributing to the development of Nursing and Midwifery policy and strategy.
  • Work with partner organisations/agencies from health and social care contributing to policy and strategy across the geographical area particularly focused on the localism and integration agendas.
  • Involvement in peer reviews of own practice and that of others.
  • Demonstrate personal duty of care towards maintenance and management of patient equipment.
  • Support the operational nursing team in effective use of financial resources.
  • Co ordinate the care of prehospital and consultant led pathways patients throughout their pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients.
  • Accountable for own actions in accordance with the Code of Professional Conduct. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
  • Lead and deliver clinical care, across a variety of settings, according to national and local guidelines ensuring delivery of evidence based care and working towards the Trust vision of providing a World Class Service.
  • Undertake an initial assessment to determine differential diagnosis, utilising advanced critical thinking and decision making skills, deciding when necessary to refer to senior clinical colleagues.
  • Act autonomously to assess, prioritise and provide expert clinical care which contributes to the patients treatment plan to ensure that optimum physical, psychological, and social care needs are met and ensures quality care.
  • Request and interpret relevant investigations to facilitate the development of the patients management plan.
  • Proactively refer to other clinicians (own or other services) depending upon patients needs to deliver care beyond your limitations and scope of practice.
  • Proactively initiate treatment plans for patients requiring intervention and treatment ensuring ethically based considerations.
  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long term conditions.
